摘 要:目的探讨老年人核性及皮质性白内障的发病机制。方法比较老年核性、皮质性白内障患者和健康人(对照组)晶状体上皮细胞(LECs)中转化生长因子β_2(TGF-β_2)mRNA、增殖细胞核抗原(PCNA)、Bcl-2/Bax、纤维连接蛋白(FN)及波形蛋白的表达,并对晶状体上皮细胞的密度进行比较。结果老年核性和皮质性白内障患者 LECs 中 TGF-β_2mRNA 的表达量与β-actin 的比值分别为0.16±0.02和0.27±0.01,与对照组0.15±0.02比较,差异有统计学意义(均为 P<0.05);两组 PCNA 蛋白的表达与内参的比值分别为16.01、1.98,与对照组2.43比较,差异有统计学意义(均为 P<0.05);两组 Bax 蛋白的表达与内参的比值分别为15.97、16.93,对照组为16.97;两组 Bcl-2蛋白表达与内参的比值分别为6.97、1.44,与对照组的6.12比较,差异有统计学意义(均为 P<0.05);免疫组化结果显示,20张切片中,核性白内障组 LECs 中 FN 阳性为1张,波形蛋白阳性为2张;皮质性白内障组分别为19张、17张,两组比较差异有统计学意义(均为 P<0.05);核性和皮质性白内障组 LECs 密度分别为(5438.40±262.03)个/mm^2和(4250.63±275.05)个/mm^2,与对照组(5368.63±211.07)个/mm^2比较,差异有统计学意义(均为 P<0.05)。结论 TGF-β_2在老年人白内障发生的过程中起重要作用。老年人皮质性白内障的发生与 LECs 的凋亡、转化有关,而老年核性白内障患者 LECs 的增殖较活跃。Objective To investigate the pathogenesis of age-related cortex and nuclear cataract. Methods The levels of transforming growth factor-β2 ( TGF-β2)mRNA, proliferating cell nuclear antigen (PCNA), ratio of Bcl-2/Bax, fihronection (FN), vimentin and the density of lens epithelial cells (LECs) were measured and compared among age-related nuclear cataract, cortex cataract and normal LECs. Results The ratio of TGF-β2 mRNA expression/β-actin expression, the levels of PCNA protein, Bax and Bcl-2 in age-related nuclear cataract(0.16±0. 02, 16. 01, 15. 97, 16. 97)and in cortex cataract(0. 27±0.01, 1.98, 16.93, 1.44) were significantly different from those in normal LECs( 0. 15±0.02, 2.43, 16. 84, 6.12), all P〈0. 05. The ratios of Bcl-2/Bax were in significant difference among 3 groups (P〈0. 05). Immunohistochemistry results showed that in all 20 slices, positive FN was found in only 1 slice, and positive vimentin was found in 2 slices in age-related nuclear cataract. In cortex cataract, positive FN , 19 slices; and positive vimentin, 17 slices. There were significant differences between the two groups (all P 〈0. 05). The density of LECs was [ ( 5438. 40 ± 262.03) entries/mm^2 ] in age-related nuclear cataract, [ (4250. 63 ± 275.05) entries/mm^2 ] in cortex cataract, and [(5368. 63 ±211.07)entries/mm^2] in normal LECs. There were significant differences among them (all P〈0. 05). Conclusions TGF-β2 may take an important part in the process of age-related cataract. Age-related cortex cataract may he related to the apoptosis and transdifferentiation of LECs. The proliferation of LECs is more active in nuclear cataract.
