p53、c-erbB-2、nm23及CEA与肺癌临床病理因素的关系

摘　　要：目的:探讨P53、c-erbB-2、nm23及CEA在未经治疗的肺癌中的表达及与临床病理因素的关系。方法:采用免疫组化法检测87例非小细胞肺癌组织及15例癌旁肺组织(距癌灶5cm)中P53、c-erbB-2、nm23及CEA的蛋白表达情况。结果:免疫组化显示P53、c-erbB-2、CEA在肺癌组织中的阳性表达率分别是45/87(51.7%)、37/87(42.5%)和41/87(47.1%),高于正常肺组织中的表达水平(P<0.05);nm23在癌组织中的表达率49/87(56.3%)低于正常肺组织的86.7%(13/15);p53的表达在不同的分化程度之间有显著性差异(P=0.043);CEA的表达在不同的病理类型之间以及在腺癌的不同TNM分期组之间有显著性差异(P<0.05);nm23表达在不同的淋巴结转移情况之间以及在不同的TNM分期之间有显著性差异(P=0.031,P=0.035);p53与c-erbB-2两者的表达有相关性(Spearman相关系数=0.319,P=0.003)。结论:p53、c-erbB-2、nm23及CEA与肺癌的发生和发展密切相关,并具有协同作用,它们的综合检测在非手术临床分期的判断及指导肺癌的临床治疗等方面具有一定价值。Objective：To investigate the expression of P53 ,c - erbB -2,nm23 and CEA in untreated lung cancer and the relationship with clinicopathologic factors. Methods：The expression of P53, c -erbB -2, nm23 and CEA was detected in the tissues paraffin - embedded from 87 cases of non - small cell lung cancer and from 15 cases of corresponding normal lung tissues of adjacent tumor as control by means of immunohistochemistry. Results： Positive rates of P53 ,c - erbB -2 and CEA in tumor tissue were 45/87（51.7% ） ,37/87（42.5% ） and 41/87（47.1% ） respectively, which were higher than that in normal controls （P 〈 0.05 ）. The positive rate of nm23 in tumor tissues was 49/87 （56.3%）, which was lower than that in normal controls, 86.7% （ 13/15）, （P 〈0.05）. There was also significant difference among the different pathologic differentiated grades for p53 expression （ P = 0.043 ） ; There was not significant difference among the different clinicopathologic factors for c - erbB - 2 expression; There was also significant difference among the different pathologic types for CEA expression （ P 〈 0.001 ）, and among the different TNM stages of adenocarcinoma for CEA expression （ P = 0. 023 ） ; There were sinificant difference among the different lymph node metastasis status and the different TNM stages for nm23 expression （P = 0. 031,0. 035, repectively）. The expression of p53 had a significant correlation with that of c - erbB - 2 （ pearson R = 0.319, P = 0.003 ）. Condu- sion：P53 ,c- erbB -2,nm23 and CEA may be involved in the process of NSCLC in coordination. Determination of P53, c - erbB - 2, nm23 and CEA expression might be of important value in diagnosis of clinical stages and instruction of treatment of lung cancer.
