溃疡性结肠炎640例结肠镜检查特点和临床分析

摘　　要：目的：总结溃疡性结肠炎（ulcerative colitis，UC）的临床特点及结肠镜下表现。方法：总结我院近10年经结肠镜检查诊断为UC的病例共640例，对其临床、内镜表现进行分析。结果：经结肠镜检查确诊的UC占同期结肠镜总例数的7．4％，男女之比为1．5：1。平均发病年龄为42岁，高峰年龄段为40～49岁。临床主要表现为腹泻、黏液脓血便、腹痛等。结肠镜表现以充血水肿（94．1％）、糜烂溃疡（75．0％）最多见，病变部位以直肠、乙状结肠为主（46．6％），呈连续性、弥漫性分布者占（89．8％）。病程小于10年者占78．0％。其中并发结肠癌或重度不典型增生9例（1．4％）。结论：本组结果显示UC有如下特点：①发病率较高，占同期内镜检查数的7．4％。②局限于左半结肠者多见。③多数病变呈连续性分布，少数为区域性分布或节段性分布。④结肠镜诊断准确率高，但仍应结合临床及病理。⑤癌症发生率低。Objective：To investigate the clinical and colonoscopic features of ulcerative colitis （UC）. Methods：640 UC patients were diagnosed by colonoscopy in our hospital in recent 10 years. Their records were retrieved and the data including.sex, age, presentation, course of disease and range of lesion were analyzed. Results： 7.4 % of cases performing colonoscopy were diagnosed as UC. The ratio of male to female was 1.5：1. The mean age at the time of diagnosis was 42 years old and the peak age was between 40 and 49 years old. The typical clinical manifestations of UC was diarrhea, bloody mucopurulent stool and abdominal pain. The common colonoscopic findings was mucosal edema and erythema （94.1% ）, erosion and ulcer （75.0 96 ）. Proctosigmoiditis was found in 46.6 % of patients. 78.0 % of patients had the course less than 10 years, including 9 patients（1.4 % ） complicated with carcinoma of colon or severe atypical hyperplasia. Conclusion：The results indicates that：①The incidence of UC was high. 7.4 % of cases performing endoscopy are diagnosed as UC. ②The lesions of UC are commonly lo- cated in the left side colon. ③Most lesions of UC are diffusive and continous. ④The diagnostic rate of colonoscopy was high,but colonoscopic findings should be combined with clinical manifestation and pathological findings. ⑤The canceration rate of UC is low.
