帕金森病患者的智能及事件相关电位研究

摘　　要：目的对帕金森病患者的智能及事件相关电位（ＥＲＰ）进行研究，并与健康者对比，以期发现其智能障碍的特点。方法用修订的韦氏成人智力量表（ＷＡＩＳ－ＲＣ）及体感刺激诱发的事件相关电位检测３１例帕金森病患者和２９例健康人，并分为老年组和非老年组，分别测得语言智商（ＶＩＱ）、操作智商（ＰＩＱ）、全量智商（ＦＩＱ）和ＥＲＰ各波的潜伏期及Ｐ３００波幅。结果５２．６％～６６．７％的帕金森病患者可出现不同程度的智能障碍，表现为皮质下痴呆的特征；其ＥＲＰ中反映认知功能活动的识别波如Ｎ２、Ｐ３００的潜伏期比健康组明显延长（Ｐ值＜０．００１），Ｐ３００波幅比健康组明显降低（Ｐ值＜０．００１）；全量智商值低于正常（≤８０～８９分）的帕金森病患者，Ｐ３００异常率明显升高（Ｐ值＜０．０１及０．０５）。帕金森病老年组和非老年组的智商、ＥＲＰ各波的潜伏期及Ｐ３００波幅之间差异均无显著性（Ｐ值均＞０．０５）。结论帕金森病患者确有不同程度的智能障碍，表现为皮质下痴呆的特征，老年组和非老年组患者智能障碍的程度无明显差别。Objective In order to find the characteristics of intellectual impairment in patients with Parkinson's disease (PD) ,a comparative study on intellingence and event-related potential (ERP)in patients with PD and healthy controls was carried out. Methods By using WAIS-RC and sensory event-related potential,VIQ, PIQ ,FIQ , the latent period of all the waves of ERP and the amplitude of P300 were measured in 31 patients with PD and 29 cases of healthy controls. The 31 patients and 29 healthy controls were divided into an aged group (≥60 years old) and a non-aged group(<60 years old). Results 52.6%-66.7% of the patients with PD had some degrees of intelligence impairment, manifested as subcortical dementia. The latent period of cognitive N2 and P300 was significantly prolonged (P<0. 001 ) and the amplitude of P300 was markedly lowered (P<0.01) as compared with these in the control group. Abnormal rate of P300 was significantly increased in PD with FIQ lower than normal (≤80-89 score, P<0.01 and 0.05). VIQ, PIQ, FIQ, the latent period of all the waves of ERP and the amplitude of P300 in the aged group of PD and non-aged group of PD had no significant difference (All P>0.05). Conclusions It is suggesed that paitents with PD do have some degrees of intelligence impairment manifested as subcortical dementia. There is no difference in degrees of intelligence impairment in the aged group and non-aged group of PD.
