摘 要:目的:分析桥粒芯糖蛋白1和桥粒芯糖蛋白2在不同表皮肿瘤中表达的改变,探讨桥粒相关成分与表皮肿瘤的临床、病理及生物学行为之间的关系。方法:采用过氧化物酶标记的链霉卵白素免疫组织化学染色方法,对桥粒芯糖蛋白1(Dsg1)、桥粒芯糖蛋白2(Dsg2)在鳞状细胞癌(SCC)、基底细胞癌(BCC)、Bowen病、日光角化病(AK)、角化棘皮瘤(KA)、脂溢性角化症(SK)中的表达进行检测。结果:桥粒芯糖蛋白1和桥粒芯糖蛋白2在正常皮肤表皮中显著表达()并广泛分布于表皮全层及毛囊、汗腺上皮;鳞状细胞癌的肿瘤组织中桥粒芯糖蛋白1、2表达显著减弱或完全消失;基底细胞癌中桥粒芯糖蛋白1、2表达显著减弱或完全消失;Bowen病、日光角化病中桥粒芯糖蛋白1、2表达下调,但与鳞状细胞癌和基底细胞癌的近核染色(+)和完全消失(-)相比,其主要表达类型为胞膜灶状染色();在绝大多数角化棘皮瘤、脂溢性角化症中桥粒芯糖蛋白1、2表达方式为强而连续的胞膜染色(),与正常皮肤表皮的表达非常相似。结论:桥粒芯糖蛋白1、2在皮肤恶性肿瘤中的表达显著减弱或完全消失,而皮肤良性肿瘤中桥粒芯糖蛋白1、2表达与正常表皮基本相同。提示皮肤恶性肿瘤中桥粒芯糖蛋白1、2的表达显著减弱或完全消失可能与肿瘤的侵袭性和远处转移有关,桥粒芯糖蛋白1、2可能对表皮良、恶性肿瘤的鉴别诊断具有一定的价值。Objective:In order to evaluate the change of desmogleins in different skin tumors and to determine how desmosomal immuno staining correlates with the clinical, biological and histopathological features of these neoplasms, we examined the expression of desmoglein 1 in various skin rumors. Methods- We investigated desmoglein 1 expression in seven types of skin neoplasms with monoclonal antibodies to desmoglein 1 used immunohistochemistry staining. Results- In normal human skin, desmoglein 1,2 was strongly expressed throughout the intercellular space of the epidermal cells, as well as the follicular epighelium and sweet glands. In SCC the expression of desmoglein 1,2 markedly reduced or absent in tumor cells. In BCC the expression of desmoglein 1,2 markedly reduced or absent in tumor cells also, Bowen's disease and paget's disease tumor cells. Whereas, it was showed that extensive pericellur staining for Dsg 1,2 was very similar to that in normal epidermis in most of KA and SK samples. Conclusions: The expression of desmoglein 1,2 is reduced or absent in variant human skin carcinomas,whereas,the expression in benign skin neoplasm is almost similar to that in normal skin. These results suggest the reduction or absence of desmoglein 1,2 excpression may contribute to the invasiveness and metastasis of skin carcinomas, the expression of desmoglein 1,2 may be of value in the defferential diagnosis of benign and malignant skin neoplasm.
