参麦注射液联合PLF方案治疗乳腺癌的临床研究

摘　　要：目的:探讨参麦注射液联合PLF(双周疗法)方案治疗乳腺癌的疗效和不良反应。方法:将74例中晚期乳腺癌患者随机分为参麦注射液联合顺铂(DDP)加大剂量醛氢叶酸(HD-LV)加5氟脲嘧啶(5-FU)持续滴注48小时的PLF治疗组35;对照组单纯用PLF化疗39例,对其疗效及不良反应进行观察比较与分析,所有病人均接受2个疗程以上的治疗。结果:治疗组有效率(CR+PR)62.9%,对照组有效率53.8%,两者间差异无显著性(P>0.05);临床受益率治疗组85.7%,对照组64.1%,两者间差异有显著性(P<0.05);治疗组和对照组中位缓解期、中位生存期分别为(9.7±3.7)月、(8.4±2.6)月(P<0.05)和(12.9±4.4)月、(11.6±4.9)月(P>0.05);治疗组治疗后与治疗前相比CD3、CD4、CD8、CD4/CD8值有明显提高(P<0.05),而对照组治疗后较治疗前下降。两组的不良反应主要是外周静脉炎、骨髓抑制、消化道反应和脱发等,其不良反应相似,两组差异无显著性(P>0.05),单纯化疗组的恶心呕吐,心脏毒性和脱发(P<0.05)、白细胞减少更多见(P<0.01)。结论:参麦注射液联合PLF方案治疗乳腺癌可以改善肿瘤患者的生存质量和提高患者的免疫功能,减轻化疗所致的血液学毒性、心脏毒性和消化道反应,值得临床推广应用。Objective：To evaluate the anti-tumor efficacy and safety of combined shenmai injection with cisplatin plus bi-weekly high-dose leucovorin and 5-fluorouracil （PLF）48-hour continuous infusion（biweek schedule） in the treatment of breast carcinoma. Methods：For January 2001 to December 2005, 74 patients with advanced breast carcinoma proved by histology were randomly divided into treatment group（35 cases） and control group （39 cases）. The treatment group received PLF（cisplatin with bi-weekly high-dose leucovorin and 5-fluorouracil 48-hour continuous infusion）plus shenmai injection therapy.The control group received PLF theray alone. All enrolled patients received two courses of treatment. Results：The effective rat of the two groups showed no marked difference （treatment group 62.9%, control group 53.8%）. The rate of clinical benefit response was 85.7% in treatment group and 64.1% in the control group,with significant difference between them（P〈0.05）. The median remission duration was （9.7±3.7）months in treatment group and （8.4±2.6）months in control group （P〈0.05）,and the median survival duration was （12.9±4.4）months in treatment group and （11.6±4.9）months in control group （P〉0.05） The values of CD3,CD4,CDs,CD4/CD8 in the treatment group significantly increased comparing with those before treatment,while the values in the control group decreased comparing with those before treatment （P〈0.05）. The major toxicity included peripheral phlebitis, bone marrow supperession,digestive tract response and trichomadesis and so on,without significant difference between two groups （P〉0.05）. Nauaea, vomiting, cardio-toxicity and trichomadesis had significant differences between two regimens（P〈0.05）. Leucopenia had very significant difference between two regimens（P〈0.01）.Conclusion：Reasonable use of shenmai injection combined with PLF（biwcck schedule）is able to improve the life quality and enhance the immune function of patients with breas
