摘 要:目的观察高强度聚焦超声(HIFU)对乳腺癌组织及其滋养血管的破坏作用。方法对23例HIFU治疗患者每例3张连续的切片分别进行HE染色、内皮细胞生物素标记的荆豆凝集素(UEA)染色光镜观察,第3张用电镜检查,观察HIFU对乳腺癌营养血管破坏的情况。结果HFIU治疗后,HE染色显示肿瘤营养血管和肿瘤组织发生凝固性坏死,UEA染色为阴性,透射电镜见乳腺癌HIFU治疗后即刻血管内皮细胞破坏,血管内血栓形成。结论HIFU使乳腺癌组织及其滋养血管完全破坏,阻止血管再生和癌组织无序增殖,从而有可能治愈乳腺癌。Objective To evaluate the effect of high intensive focused ultrasound (HIFU) on breast carcinoma angiogenesis. Methods The target lesions in 23 breast cancer patients treated by HIFU was observed after autopsy, one slide for HE staining, one for vascular endothelial cell biotinylated-ulex europaeus agglutinini- I (UEA I ) immunohistochemical staining, both of them for microscopic observation; another one for electron-microscopic observation. Results Histological examination of HE staining indicated HIFU made tumor tissue and its vascularities coagulative necrosis but UEA I with immunohistochemical staining of the target lesions was negative. After HIFU irradiation, tumor capillary ultrastructure was destroyed immediately, membrane architecture was broken, fundus membrane was unclear ; thrombosis and endothelial destruction were observed under transmission electron microscope examination. Conclusions HIFU may destroy breast carcinoma as well as its vascularities thoroughly. It may inhibit breast cancer cells and its new vascular genesis out of control, which makes HIFUVs tumor treatment more effective.
