皮质下动脉硬化性脑病病人颈动脉超声的研究

摘　　要：目的研究皮质下动脉硬化性脑病(SAE)病人的颈动脉粥样硬化特点及血流动力学变化。方法应用彩色多普勒超声仪,检测42例SAE病人双侧颈总动脉(CCA)、颈内动脉(ICA)、颈外动脉(ECA)粥样硬化斑块发生情况,观察收缩期峰值血流速度(SPV)、舒张末期血流速度(EDV)和阻力指数(RI)变化,并与40例年龄匹配、伴随疾病相似、无明显缺血性心脑疾病症状、体征的病人进行比较。结果①SAE组颈动脉内膜-中层厚度(IMT)明显增加,与对照组比较有统计学意义(P<0.05);②SAE组斑块检出率97.62%,对照组斑块检出率82.50%,两组比较有统计学意义(P<0.05);③粥样硬化斑块积分SAE组明显高于对照组(P<0.01);两组间管径狭窄皆以轻度狭窄(<50%)为主,但SAE组中度、重度狭窄(>50%)明显高于对照组(P<0.05);④SAE组SPV、EDV较对照组明显减慢(P<0.05)。结论SAE病人颈动脉粥样硬化多较显著,血流动力学改变以流速减慢为主。SAE的发病与颈动脉粥样硬化有关。Objective To investigate the features of carotid atherosclerosis and the hemodynamics in patients with subcortical atherosclerotic encephalopathy. Methods Forty - two patients （pts） with subcortical atherosclerotic encephalopathy （SAE group） were examined by carotid Doppler. The incidences of atherosclerotic plaques in bilateral carotid artery （CCA）, internal carotid artery （ICA） and external carotid artery （ECA） and the hemodynamical parameters including systolic peak velocity （SPV）, end- stage diastolic velocity （ECV） and resistance index （RI） were analyzed. Results ①The intima - media thickness of carotid wall in SAE group was significant thick comparing to that in control group（P 〈 0.05）. ②The incidence rate of atheroselerotic plaque was 97.62% in SAE group and 82.50% in control group （P〈0.05）. ③The majority of pts in two groups showed mild degree of carotid stenosis （ 〈 50 % ）. The pts with middle to severe degree of carotid stenosis （ 〉 50% ） in SAE group were more than that in control group（P 〈 0.05）. ④ The SPV and EDV in SAE group were significantly decreased than that in control group（ P 〈 0.05）. Conclusion The incidence of carotid atheroselerosis was more frequent, the degree was severe and main index of hemodynamics was worse in pts with SAE.
