咽部定植菌与住院精神病人呼吸道医院感染相关性的研究

摘　　要：目的探讨咽部定植菌与住院精神病人呼吸道医院感染的相关性及预防控制对策。方法将符合入组条件的新入院精神病人纳为研究对象,分别在入院后24 h、第4 d、第21 d时取咽拭子送检做细菌培养和药敏试验,并对其住院期间的医院感染情况进行统计分析。结果咽部有病原菌定植组病人呼吸道医院感染率达53.84%,与无定植组的16.98%比较,有极显著差异(P<0.01),定植菌耐药情况严重。结论咽部病原菌定植是住院精神病人呼吸道医院感染的主要潜在危险因素之一,抗精神病药物的副反应和约束护理是引起高感染率的主要原因;临床应重视咽拭子培养和药敏结果来指导预防和治疗住院精神科病人呼吸道医院感染。objective To explore the relationship between the bacteria colonized in oropharynx in psychopath and nosocomial infection of respiratory tract,and preventing controlling countermeasure. Methods To selected 105 psychopath in our hospital,pharyngeal swab cultivate on 24 h,4 day and 21 day after in hospital,than analyzed the nosocomial infection of respiratory tract in their. Results Bacteria colonized in oropharynx in psychopath the group＇s rate of nosocomlal infection of respiratory tract was 53.84% ,the relative ratio（RR） was 3.76,it had significantly different （P〈0.01）. Drug-fast was serious in these bacteria that colonized in oropharynx in psychopath. Conclusion The bacteria colonized in oropharynx in psychopath is one of the primary dangerous in nosocomial infection of respiratory tract, the side-effect of the drug and binding nursing in psychopath is one of the primary reason. Pharyngeal swab cultivate can be useful in therapeutic of nosocomial infection of respiratory tract in psychopath.
