组织蛋白酶B在卵巢癌中的表达及意义

摘　　要：目的:探讨卵巢癌组织中组织蛋白酶B(cathepsinB,CB)的表达及其与微血管密度(MVD)、癌细胞增殖标记指数(Ki67标记)的关系。方法:采用免疫组化链霉菌抗生素蛋白过氧化物酶连接法(SP法)检测10例正常卵巢,13例卵巢良性肿瘤和73例卵巢癌组织中CB,CD34和Ki67的表达,并结合临床、病理及随访资料进行分析。结果:CB在正常和良性卵巢组织中不表达,在卵巢癌组织中CB阳性表达率为52.1%(38/73)。CB表达阳性和阴性的卵巢癌组织中,MVD的平均值分别为37.62±11.42,23.51±8.14(P<0.05);Ki67标记指数分别为59.34±17.53,44.60±12.71(P<0.05)。卵巢癌组织中CB的阳性表达与年龄、病理类型和组织学分化等级无关(P>0.05),与FIGO分期、腹水和转移有关(P<0.05)。CB表达阳性的生存期低于阴性者。结论:CB阳性表达与生存率有关,可能是卵巢癌的不良预后因素。Objective： To study the expression of cathepsin B （CB） in ovarian carcinoma and its correlation with microvessel density （MVD） and K167 proliferation index. Methods： Streptavidin peroxidase immunohistochemical method was used to examine the expression of CB,CD34,and Ki67 in 10 cases of normal ovarian tissues,13 cases of benign ovarian tumors,and 73 cases with ovarian carcinoma. The results were analyzed combined with clinical and pathological condition and follow-up data. Results： The expression of CB was not detected in normal ovary tissues and benign ovarian tumors. But the positive rate of CB was 52. 1% （38/73）in ovarian carcinoma tissues. In the Ct3-positive and CB-negative ovarian carcinoma tissues,the average value of MVD was 37.62 ± 11. 42 and 23.51 ± 8.14（P〈0.05） and that of Ki67 was 59.34 ± 17.53 and 44.60 ±12.71 （P〈0.05）, respectively. The positive expression of CB was not correlated with the age, pathological classification, histological differentiation degree. But it was correlated with FIGO stage,ascitic fluid,and metastasis. CB-positive patients had a significantly shorter survival time than CB-negative patients. Conclusion：Positive expression of CB was closely associated with survival rate and may be related to worse prognosis.
