摘 要:目的比较氯丙嗪与利培酮对人体姿态平衡的效应。方法对象为79例精神分裂症患者,其中40例用氯丙嗪治疗,39例用利培酮治疗;两组分别在治疗第3天与第28天时,应用人体姿态平衡仪检测,以及在入组与治疗第28天时,分别采用简明精神病量表测评。结果①在治疗第3天,氯丙嗪组(100mg/d)的路径总长比利培酮组(1mg/d)明显为长[(1052±298)mm:(795±160)mm,P<0.001];Romberg路径比比利培酮组明显为大[(1.26±0.25):(1.14±0.18),P<0.05]。②在治疗第28天时,氯丙嗪组[(386.7±86)mg/d]的外围面积和Romberg面积比比利培酮组[(3.6±0.9)mg/d]明显为大[(483±303)mm2:(351±199)mm2;(1.6±0.7):(1.2±0.6),P均<0.05];路径总长也显著大于利培酮组[(1090±258)mm:(734±144)mm,P<0.001]。③两组在治疗第3天和第28天时,平衡试验参数与精神病症状之间均无显著相关性(P>0.05)。结论在第3天低剂量治疗时,氯丙嗪组对姿态平衡功能的损害大于利培酮;在第28天治疗量时,氯丙嗪组对平衡功能的损害进一步大于利培酮组。平衡功能的改变与精神分裂症的症状之间无关。Objective To compared ehlorpromazine with risperidone effect on body posture balance. Methods Body posture balance test was estimated in 40 schizophrenics taking chlorpromazine and 39 schizophrenics taking risperidone at posttreatment 3^rd and 28^th day, respectively. BPRS was also estimated in the chlorpromazine group and the risperidone group at the pretreatment and posttreatment 28^th day, respectively. Results At postteatment 3^rd day, track total distance was significantly longer in patients taking chlorpromazine(100mg/d) than patients taking risperidone (1 mg/d) [ (1052 ± 298 )mm vs (795 ± 160) ram, P 〈 0. 001 ] and it was significantly larger for ratio of Romberg track distance in patients taking chlorpromazine than patients taking risperidone [ ( 1.26 ± 0. 25 ) vs ( 1.14 ± 0. 18 ), P 〈 0. 05 ]. At posttreatment 28^th day, the surround area and the ratio of Romberg area ( blind the eyes/open the eyes) were significantly larger in patients taking chlorpromazine [ (386.7 ± 86) mg/d] than patients taking risperidone[ (3.6±0. 9) mg/d] [ (483 ± 303) mm^2 vs(351 ± 199)mm^2 ,(1.6 ± 0. 7)vs ( 1. 2 ± 0. 6 ), P 〈 0.05 ] ;track total distance was also significantly longer in patients taking chlorpromazine than patients taking risperidone [ ( 1090 ± 258 )mm vs( 734 ± 144) ram, P 〈 0. 001 ]. There were no significant correlations between psychotic symptoms and the parameters of balance test at posttreatment 3^rd and 28^th day in two groups. Conclusion At posttreatment 3^rd day, there was more damage for posture balance function in patients taking chiorpromazine than patients taking risperidone;At posttreatment 28^th day, the damage was more prominent; There was no relationship between the damage of balance function and schizophrenic symptoms.
