摘 要:目的探讨IGF-1、bcl-2和ki-67蛋白在星形胶质细胞反应性增生与低级别星形胶质细胞瘤中表达的区别及意义。方法应用组织芯片和免疫组化染色技术检测正常脑组织、星形胶质细胞反应性增生、低级别(Ⅰ~Ⅱ级)和高级别(Ⅲ~Ⅳ级)星形胶质细胞瘤中IGF-1、bcl-2和ki-67蛋白的表达情况。结果正常脑组织中三者均为阴性表达;星形胶质细胞反应性增生组中IGF-1、bcl-2和ki-67阳性率分别为28.9%、26.7%、22.2%;低级别星形胶质细胞瘤组中IGF-1、bcl-2和ki-67阳性率分别为63.8%、50.0%、70.2%;高级别星形胶质细胞瘤组中阳性率分别为88.9%、79.2%、95.2%。IGF-1、bcl-2和ki-67在各实验组间比较均差异显著(P〈0.01);IGF-1、bcl-2和ki-67表达与组织学分级密切相关(r=0.997、r=0.999和r=0.988,P〈0.01);IGF-1与bcl-2、ki-67表达密切相关(r=0.995和r=0.995,P〈0.01)。结论IGF-1和bcl-2在星形胶质细胞瘤的发生、发展中起重要作用;IGF-1、bcl-2和ki-67联合应用在星形胶质细胞反应性增生与低级别星形胶质细胞瘤的鉴别诊断中可能会有一定的意义。Objective To study the difference of IGF-1 、bcl-2 and ki-67 protein expression in reactive and neoplastic astrocytes and the significance of them. Methods Irnmunohistochemistry and tissue microarray techniques were used to determinate the expression of IGF-1、 bcl-2 and ki-67 protein in 12 cases of normal brain tissues, 49 cases of astrocytes proliferation, 49 cases of low-grade astrocytomas, and 50 cases of high-grade astrocytomas, 12 cases of normal brain tissues were used as controls. Results The expression of IGF-1, bcl-2 and ki-67 protein were all negative in control group; The positive rates of expression of them in reactive astrocytes were 28. 9% ,26. 7% ,22. 2% respectvely; and in low-grade astrocytomas were 63. 8%, 50. 0%,0, 70. 2%; in high-grade astrocytomas were 88. 9%,79. 2%,95. 2%. There were all significant difference among the three experimental groups(P〈0. 01) ; the expression of IGF-1. bcl-2 and ki-67 protein had close correlation to the histological grades (r= 0. 997, r = 0. 999, r= 0. 988, P〈0. 01) ; the expression of IGF-1 was positively correlated to bcl-2 and ki-67 protein(r= 0. 995 or r= 0. 995, P〈0. 01 ). Conclusion IGF-1 and bcl-2 protein plays an important role in the progression of astrocytomas; IGF-1 ,bcl-2 and ki-67 protein might be combined utilized to discriminate between reactive astrocytes proliferation and low-grade astrocytomas.
