采用免疫细胞化学技术进行急性白血病分型及骨髓转移癌鉴定的意义

摘  要:目的探讨急性白血病(AL)免疫分型及肿瘤细胞骨髓转移的鉴定。方法采用免疫细胞化学法,使用常规8种CD分子抗体和二种CK分子抗体,测定39例AL免疫表型和10例肿瘤细胞骨髓(BM)转移的免疫表达。结果免疫分型与FAB形态学分型有较高的符合率,也存在CD抗原在淋系或髓系交叉或反向表达。3例AL应用FAB形态学难以定型的,应用免疫细胞化学得到了鉴定。10例肿瘤细胞骨髓转移应用免疫细胞化学得到了诊断。结论应用免疫细胞化学可以弥补FAB形态学对白血病分型的不足,尤其是应用FAB形态学难以分型的AL及肿瘤细胞骨髓转移的鉴定,提高其诊断的准确性。Objective To Study the inmmunity typing of acute leukemia (AL) and the identification of metastatic tumor bone in ne marrow, Methods The immunity implication assays inmmunity phenotypes of 39 cases acute leukemia and 10 cases bone marrow's (BM) metastatic tumor cell with the two-stages of inmmunohisto- chemistry labeled with horse radish peroxidase, 8 routine anti- CD molecular anti body and 2 kinds of anti CK molecular anti - body. Results The immunity typing and FAB morphological classification manifest a high coincidental rate. However, there were CD antigen's cross and reversal phenomena in lymphatic system and bone marrow system, 3 cases of acute leukemia which were difficult to type with FAB morphology had been identified with immunohisto- chemistry, 10 cases of bone marrow's (BM) metastatic tumor cell were also diagnosed with inmmunohisto chemistry. Conclusions The insufficiency in leukemia typing with FAB morphology can be supplemented with inmmnohisto - chemistry, especially those acute leukemia and metastatic tumor in bone marrow which are difficult to type with FAB morphology so as to enhance their accuracy.
