癌标志物联合测定对鉴别良恶性结肠直肠疾病的价值

摘　　要：对结肠直肠疾病患者血清癌胚抗原(CEA)、糖链抗原19-9(CA19-9)和50(CA50)水平进行了测定。发现恶性疾病组的肿瘤标志物水平明显高于良性疾病组;还发现肿瘤标志物联合测定阳性者(CEA≥15μg/L、CA19-9≥37kU/L和CA50≥17kU/L三项中的两项超过临界值或单项超过其两倍以上者)进展期癌组占96.67％(29/30例),在炎性疾病组全部阴性,故推荐以此作为两者的鉴别标准。而癌前病变和早期癌两组的肿瘤标志物单项阳性者差别不显著,故对单项阳性或肉眼形态可疑癌变者,应进行内镜活组织复查以便使癌早期发现。The serum levels of carcinoembryonic antigen (CEA) .carbohydrate antigens 19-9 (CA19-9) and 50 (CA50) were determined in patients with benign and malignant colorectal diseases. It were found that the levels in malignant colorectal disease groups were strongly elevated than the levels in benign colorectal disease groups, and that the combinating positive of cancer markers (the values were exceeded two or three items:CEA>15μ/L,CA19-9>37kU/L,and CA50>17kU/L) oc-coured in 96. 67%(29/30) of advanced cancer groups,all negative occoured in 96- 67%(20/30)of inflammatory disease groups, which are recommended for the differential criterions of inflammatory disease and advanced cancer. There were not significant differentiation between precancerous change and early carcinoma,so endoscopic biopsies were undertaken to detect early carcinomas in patients with tumor marker-positive or suspecting canceration-naked shapes.
