血清肿瘤坏死因子α测定对消化道癌诊断的评价

摘　　要：本文报告61例消化道癌、30例消化道非肿瘤疾病和50例正常人三组血清TNFa测定结果:分别为14.59±3.01ng/ml、2.31±0.56ng/ml和0.05±0.04ng/ml,消化道癌组显著高于其他两组(P均<0.01)。42例未切除肿瘤的血清TNFa水平为15.94±4.01ng/ml,显著高于19例手术切除者8.31±3.91ng/ml(P<0.01);23例有转移者16.24±4.37ng/ml又显著高于19例无转移者7.76±5.29ng/ml(P<0.01)。以血清TNFa水平3.4ng/ml为切割值,诊断消化道癌的敏感性、特异性和正确性分别为80.3％、83.3％和81.3％。消化道癌中以大肠癌血清TNFa水平最高,为25.54±6.41ng/ml,诊断敏感性为92.6％。本文认为测定血清TNFa对道化道癌的诊断和判断病情是有价值的。:This paper reported that the serum TNFa were determined in 3 groups: 61 patients with diges-tive tract carcinoma, 30 patients wi benign digestive tract diseases and 50 normal controls and its resalts were14.59± 3. 01, 2. 31 ±0. 56 and 0. 05± 0. 04ng/ml respective1y. The serum TNFa level in the patients with di-gestive tract carcinoma was significantly higher than those of other two groups (both Pwt0. 0l ). The serumTNFQ ieve1(15. 94± 4. 01ng/ml) in 42 case of tumor unresected group was significantly higher than those of(8. 31 ±0. 91ng/ml) in 19 case of resected group,and 16. 24±4. 37ng/ml in 23 case of metastic group was sig-nificantly higher than those of (7. 76± 5. 29ng/ml) in 19 case of non-metastatic group. If serum TNFa level 3.4ng/ml was regarded as cut off value,its sensivity.specificity and accuracy in the diagnosis of digestive tractcarcinoma were 80. 3% .83. 3% and 81. 3% respectively. The serum TNFa level (25. 54± 6. 14ng/ml) in thepatients with large intestinal carcinoma was the highest,and its diagnositc sensitivity was 92, 6 %. We suggestthat the determination of TNFa has clinical significance in the diagnosis and judgment of the advance the dis-ease.
