41例根管外科治疗的预后分析

摘　　要：目的:总结经验以期能更好的为根管外科治疗提供临床资料。方法:选择有记载的有随访的根管外科治疗的病例41例共41颗牙齿,分别根据所采取的手术方式分4组,分别是根尖刮治术组12例,根尖切除术组10例,根尖倒充填术组5例和根尖切除+根尖倒充填术组14例。结果:41例患者中,治愈30例,好转5例,失败6例,总成功率为85.4%。4组的治疗成功率分别是:根尖刮治术组66.7%(8/12),根尖切除术组90%(9/10),根尖倒充填术组80%(4/5)和根尖切除+根尖倒充填术组100%(14/14)。根尖切除+根尖倒充填术组疗效优于单纯根尖刮治术组。结论:牙根长度足够的情况下尽量采取根尖切除术+根尖倒充填术联合治疗顽固性根尖周炎或戴有修复体的基牙的根尖周炎。进行根管外科治疗是一种简单,经济,有效的辅助治疗手段。Objective：To provide to clinical data for endodontic surgery. Methods： Case records of 41 teeth of 41 patients who performed by endodontic surgery with chronic perioapical periodontitis were divided to 4 groups in which there were 12 cases of periapical curettage, 10 cases of apicetomy, 5 cases of retrograde filling and 14 cases of apicetomy ＋ retrograde filling, respectively. Results： Of 41 cases , 30 cases were healed, 5 improved and 6 failed, with a total successful rate of 85. 4%（35/41）, and the successful rates of 4 groups were 66.7% （8/12） for periapical curettage, 90% （9/10） for apicetomy, 80% （4/5） for retrograde filling and 100%（14/14） for apicetomy ＋ retrograde filling, with the group of apicetomy ＋ retrograde filling superior to the group of periapical curettage. Conclusion： In chronic perioapical periodontitis which has to be treated by endodontic surgery, it is better to choose apicetomy ＋ retrograde filling for the tooth that has enough root length. Endodontic surgery was a kind of simple, economic and effective assistant treatment.
