伊立替康联合顺铂和5-FU治疗晚期胃癌

摘　　要：目的评价伊立替康(CPT-11)联合顺铂和5FU治疗晚期胃癌的疗效与毒副反应。方法将50例晚期胃癌患者分成两组,治疗组25例采用ICF方案:CPT-11 180 mg/m2静脉滴入90 min d1;顺铂70 mg/m2静脉滴入d1;5-FU 400mg/m2持续静脉滴入d1-d5,每4周重复,至少两个周期;对照组25例采用CF方案:顺铂70 mg/m2静脉滴入d1;5-FU 400 mg/m2持续静脉滴入d1-d5,每4周重复至少两个周期。结果所有病人均可评价。治疗组有效率为52%,中位生存期为10.25个月,一年生存率为44%;对照组有效率为24%,中位生存期为7.45个月,一年生存率为16%,以上两者在有效率,总生存及一年生存率之间均有显著差异。治疗组在腹泻、骨髓抑制发生率方面明显高于对照组,但经过相应处理后均能控制。结论伊立替康联合顺铂和5-FU治疗晚期胃癌疗效确切,不良反应可以耐受,值得进一步研究和推广。Objective To evaluate the efficacy and toxicity of irinotecan combined with cisplatin and 5-Fluorourcil（ICF）in the treatment of advanced gastric cancer. Methods Fifty cases were divided into two groups. In therapeutic group 25 eases received ICF regimen. CPT-11 180 mg/m^2 ivgtt 90 minutes dl ,DDP 70 mg/m^2 ivgtt dl ;5-FU 400 mg/m^2 ivgtt 24 h d1-d5 ,and repeated every 28 days,two cycles at least;In control group 25 cases received CF regimen（cisplatin and 5-fluorourcil） DDP 70 mg/m^2 ivgtt dl ;5-FU 400 mg/m^2 ivgtt 24h d1-d5,and repeated every 28days, received two cycles at least. Results In the therapeutic group：Objective response rate of 52% was achieved. Median survival was 10.25 months with 1-year survival vate of 44%. In contrast,in the control group： Objective response rate of 24% was achieved and median survival was 7.45 months with 1-year survival rate of 16%,and there was significant difference between them. The toxic reations such as diarrhea and myelosuppression in therapeutic group were much higher than those in the control group. The toxic creations, however, could be controlled with relative thra- py. Conclusion ICF regimen is effective and well tolerable for advanced gastric cancer,and it is worth to be used and researched widely.
