摘 要:目的根据女性体细胞中两条X染色体上雄激素受体(AR)基因的长度多态性进行非放标克隆性检测,以探讨肝脏海绵状血管瘤的克隆组成。方法用石蜡切片,HE染色后进行显微解剖,提取基因组DNA,经HhaⅠ消化后,巢式PCR扩增,变性聚丙烯酰胺凝胶电泳后银染显示单股DNA片段长度。结果在5例肝脏血管瘤标本中,4例具有AR位点多态性,每例取不同部位肿瘤组织标本12个,肿瘤周围对照组织标本4个,共64个标本,其中46个肿瘤组织标本和所有对照标本扩增成功,所有对照标本均显示为多克隆性增生,而46个肿瘤标本中,43个标本为单克隆性增生(93.5%),3个标本为多克隆性增生(6.5%)。结论肝脏海绵状血管瘤均为克隆性增生,属于真性肿瘤。Objective:To demonstrate the clonality status of cavernous hemangioma of liver using an assay based on inactivation mosaicism of the length - po]ymorphic X chromosomes at the androgen receptor (AR) locus. Methods: Twelve tumor component from different palace of each liver hemangioma were collected using micredissection from the routine sections stained with hematoxylin and eosin. Four Stroma components were also isolated from the same samples, and used for parallel controls. Genomic DNA was extracted, pretreated with Hha I, and amplified via nested PCR. The products were resolved on denaturing polyacrylamide gels and visualized through silver staining. Results: A total number of five cases were examined, four cases carrying the length polymorphism at the AR locus. Fortysix tumor components and all stroma components were successfully amplified. All stroma components were shown to be polyclonal. Loss of X - chromosome inactivation mesaicism was found in fortythrec tumor components, reflecting their monoclonal cell composition. The same inactivated allele was found in all tumor components from the same case, reflecting all tumor components from the same case have originated from a single cell. Condusion: In cavernous hemangioma of liver, most of tumor components are monoclonal, and all tumor components from the same case have originated from a single cell. They were already neoplastic.
