强力霉素滴眼液对兔蒸发过强型干眼疗效的评价

摘　　要：目的评价强力霉素滴眼液对兔蒸发过强型干眼的疗效,并初步探讨其机制。方法新西兰大白兔16只,烧灼睑板腺开口制作干眼模型,术后第6周,随机分为4组,分别不滴药及滴用1%甲基纤维素、0.1%强力霉素、1%甲基纤维素加0.1%强力霉素。用药前后定期检查角膜荧光素染色、虎红染色、结膜杯状细胞密度等。术后第11周取眼睑、结膜及角膜,行病理学检查。结果用药前,各组兔角膜荧光素染色、虎红染色、结膜杯状细胞密度等指标无统计学差异(P<0.05)。用药后,甲基纤维素与强力霉素联合应用组荧光素染色及虎红染色比未用药组轻,结膜杯状细胞密度比未用药组高(P<0.05)。单纯使用甲基纤维素或强力霉素组上述指标与未用药组比较无显著性差异(P>0.05)。病理学检查显示,未用药组睑结膜及球结膜可见炎症细胞浸润,其他组未见明显的炎症细胞浸润。结论强力霉素滴眼液可以减轻干眼兔眼表的炎症反应,其与甲基纤维素联合应用对兔蒸发过强型干眼有一定的治疗效果,提示强力霉素对干眼的治疗具有一定的应用前景。Objective To study the effect and mechanism of doxycycline eye drops on evaporative dry eye. Methods Sixteen New Zealand albino rabbit models were establisbed by oauterizing the. meibomian gland orifices, then divided into four groups on the sixth post-operative weeks. The 1, 2, 3 group was respectively treed 1% hydroxyethylcellulose eye drops,0. 1% doxycycline solution, and 1% hydroxyethylcellulcse eye drops added 0.1% doxycycline solution four times every day. The forth group was not treated as control. Eyelid swelling, lid margin hyperemia, lid margin eschar, meibomian gland orifices, conjunctival hyperemia, rose bengal .,stain, fluorescence stain,Schimer Ⅰ test and conjtmcfiva impression cytology test were performed before and after operation. After 11 weeks eyelid, conjunctiva, and cornea tissues biopsy was carried out under optical microscope. Results Before using eye drops, no significant difference was found among four groups in rose bengal stain, fluorescence stain and conjunctival goblet cell density. On 1, 2, 3, 5 weeks after dripping eye drops, rose bengal stain and fluorescence. by 1% trydroxyehylcellulose eye. drops added 0.1% doxycycline solution was lower than by 0.9% sodium chloride solution, and goblet cell density was higher than by 0.9% sodium chloride solution （ P 〈 0.05）. No significant difference was observed among other three, groups （ P 〉 0.05）. Under optical microscope examination of pathologic .,dice, inflammation cell could be found in conjunctiva tissues hiopsy of control group. Conclusion Doxycycline eye drops can alleviate the ocular surface inflammation of dry eye rabbit, this eye drops combining artificial tear can decrease dry eye. signs which suggests doxycycline eye drops is valuable to dry eye.
