摘 要:目的:研究心力衰竭(CHF)大鼠吸入磷酸二酯酶Ⅲ抑制剂——米力农后血流动力学指标变化。方法:设充血性心力衰竭(CHF)组:雄性sD大鼠24只(100±8)g,在第二、三肋间开胸,用1mm肽夹钳夹主动脉,造成主动脉缩窄,8~10周后,造成CHF,大鼠至(350±10)g,随机分为①吸入NaCI3min,②吸入米力农(1mg/m1)3min后予第二次开胸。正常对照组:雄性SD大鼠24只(350±10)g,随机分为①吸入NaCI3min,②吸入米力农(1ms/m1)3min。两组大鼠均予颈动、静脉置管,开胸后分别给予肺动脉和左房置管,分别测量平均动脉压(MAP)、肺动脉压(PAP)、左房压(LAP)、中心静脉压(CVP)和心排出量(CO),并计算肺血管阻力(PVR)、外周血管阻力(SVR)、总血管阻力(TVR)、PVR/SVR,测量血气和称量肺干湿重比(W/D)和心脏重量和体重比(HW/BW)。结果:①CHF大鼠吸NaCI前后PVR/SVR无明显改变;②CHF大鼠吸米力农前后PVR/SVR明显下降,P<0.05;③对照组吸米力农前后PVR/SVR无明显改变;④CHF大鼠HW/BW明显高于正常对照组大鼠,P<0.05;CHF大鼠肺W/D明显高于正常对照组大鼠,P<0.05。结论:米力农吸入治疗明显降低CHF大鼠PVR/SVR,吸入治疗不改变体循环血压而降低肺动脉压,这种方法可作为新的治疗CHF的方法用于临床。Objective: To study changes in hemodynamics of milrinon inhalation treatment congestive heart failure(CHF) rats. Methods: CHF rats group: male 24 SD rats( 100 ± 8 ) g, bw, in the first open chest operation first open chest operation supracoronary aortic banding with titanium clip was giving, progressive development of CHF over 8 - 10 weeks, the rats weight arrived (350 ± 10)g; In the second operation, put the catheter in pulmonary artery and left atrium,randomly divided two groups with each group 12 rats: group 1 inhalation NaC1,3 rain, group 2 inhalation milrinone ( 1 mg/ml) ,3 rain. Control group : normal male 24 SD rats (350 ±10)g,bw, randomly divided two groups, group 1 inhalation NaC1,3 rain, group 2. inha- lation milrinone( 1 mg/ml) ,3 rain,and were put the catheter into pulmonary artery and left atrium too. Both groups we measured MAP, PAP, LAP, CVP CO and calculated PVR, SVR, TVR, PVR/SVR. We meas- ured blood gas and calculated the ratio of wet/dry lung weight(W/D) ,the ratio of heart/body weight( HW/ BW). Results: ①In CHF rats before and after inhalation NaC1 the ratio PVR/SVR had no significant change ; ②In CHF rats before and after inhalation mirinone the ratio PVR/SVR had significant decrease, P 〈0. 05 ;③ In control group rats before and after inhalation mirinone the ratio PVR/SVR had no significant change ; ④In CHF rats the ratio of HW/BW was significantly increased P 〈0. 05, and the ratio of lung W/ D was significantly increased too, P 〈0. 05. Conclusion : Milrinon inhalation treatment decreased the ratioPVR/SVR in CHF. this treatment was a new trials to treating pulmonary venous hypertension.
