功能性消化不良者胃排空功能与症状的相关研究

摘　　要：目的探讨功能性消化不良(FD)患者的胃排空功能变化及其与FD主要症状的关系。方法双核素标记试餐SPECT检测胃排空功能,并对22例患者的症状进行分级评估。结果 FD患者液体胃排空和胃内分布模式与对照组相似;固体的排空模式与对照组不同,表现为先快速后缓慢的排空,12例不存在延迟相(P<0.05)。固体半排空时间延长者11例,液体半排空时间延长者2例,两者均延长者2例。摄食60min后远端胃内固体容量大于对照组。但这些排空和分布改变与FD之主要症状均无显著相关性。结论 FD患者以固体食物排空异常为主,胃排空和/或胃内分布异常不能完全解释FD,这种食物胃内行为变化与症状无明显关系。AIMS To quantify the partition of gastric contents between proximal and distal stomach during gastric emptying and evaluate the relationship between gastric emptying function and functional dyspepsia(FD) symptoms. METHODS Twenty-two patients with FD and 17 healthy volunteers ingested a standard meal labbled with ^(99m)Tc and ^(113m)In.Serial images of gastric area were taken by SPECT-scinitgraphic techniques, regions of interest for proximal, distal, and total stomach were defined, and activity time curves were derived. RESULTS The emptying pattent and half time(T_(50)) for liquid in patients with FD were not significantly different from controls. In the patients, emptying pattent for solid has been changed and distinguished from that in controls. Eleven showed delayed T_(50) for solid, two for liquid and two for both. Lag phase before emptying was disappeared in 12 patients(P<0.05). Contents of solid in the distal region after 60 min was significantly higher than in controls (P<0.05) during emptying. However, there were not significant differences in intragastric distribution between patients with and without delayed T_(50). The changed pattent and delayed emptying for solid were not related with the major symptoms, such as abdominal pain( P>0.20), bloating(P>0.20). CONCLUSIONS FD patients show delayed emptying and/or maldistribution, but not being attributed to their symptoms.
