中下段直肠癌血管内皮生长因子表达与直肠系膜转移的关系

摘　　要：目的分析中下段直肠癌血管内皮生长因子(VEGF)表达与临床病理特征的关系,初步探讨中下段直肠癌直肠系膜转移的分子机制。方法采用病理大切片前瞻性研究56例中下段直肠癌直肠系膜转移情况,采用免疫组织化学技术检测肿瘤组织VEGF表达。结果57．1% (32/56)中下段直肠癌VEGF表达阳性；T3直肠癌VEGF表达阳性率为74．1%,明显高于T2和T1直肠癌的43．5%和33．3%(P＜0．05)；淋巴结转移阳性的中下段直肠癌VEGF表达阳性率为72．4%明显高于淋巴结转移阴性的40．7%(P＜0．05)；中下段直肠癌直肠系膜转移率为64．3% (36/56)。36例系膜转移阳性直肠癌25例(69．4%)VEGF表达阳性,而20例系膜转移阴性直肠癌仅7例(35%)VEGF表达阳性,两者差异有统计学意义(P＜0．05)。结论中下段直肠癌VEGF表达与浸润深度和淋巴结转移密切相关。VEGF可能参与中下段直肠癌直肠系膜转移的发生。Objective To study the expression of VEGF in middle and lower rectal cancer and its correlation with mesorectal metastases. Methods The resected primary tumors from 56 patients with middle and lower rectal cancer who received total mesorectal excision at the Department of General Surgery of Guangdong Provincial People＇s Hospital were studied. Results The VEGF expression was positive in 32 （57.1% ） specimens. 74.1% T3 tumors were positive for the VEGF expression, while only 33.3 % T1 tumors and 43.5 % U2 tumors were positive for VEGF expression, P〈0.05, 72.4 % （21/ 29） rectal carcinomas with positive lymph nodes were positive for VEGF expression, while only 40.7 % rectal carcinomas with negative lymph nodes were positive for the VEGF expression, P〈0.05. Mesorec- tal metastases were detected in 36 （64.3 % ） of 56 cancer specimens. The expression of VEGF was positive in 25 （69.4%） of the 36 patients with mesorectal metastases, and in 7 （35%） of the 20 patients without mesorectal metastases. The difference between these two groups was statistically significant （P〈0.05）. Conclusion The expression of VEGF in middle and lower rectal cancer specimens is significantly associated with depth of tumor invasion and lymph node metastasis. The high expression of VEGF may play an important role in the development of mesorectal metastases in patients with middle and lower rectal cancer.
