血清CA_(125)测定对卵巢恶性肿瘤的诊断及随访复发的价值

摘　　要：本文采用放射免疫法对98例165份血清进行糖类抗原CA_(125)。值的测定,其中正常对照组30例,卵巢良性肿瘤25例,交界性肿瘤1例,卵巢恶性肿瘤42例。对恶性肿瘤患者中24例同时进行了经阴道超声检查的随访监测。发现卵巢恶性肿瘤患者血清CA_(125)值明显高于良性肿瘤和正常对照组(p〈0.01);良性肿瘤CA_(125)值与正常对照组无显著性差异(p〉0.05),术前卵巢恶性肿瘤阳性检出率85.19％,其中卵巢上皮性癌阳性诊断率90％;术后卵巢恶性肿瘤CA_(125)值明显低于术前值(p〈0.01),逐步回归分析表明:残余瘤灶是术后CA_(125)。回归方程中最主要的决定因素(p=0.0197),CA_(125)的测定,对诊断早期复发的敏感性100％,特异性93.75％。 提示CA_(125)是一项特异性较高的卵巢恶性肿瘤标志物,对良、恶性肿瘤的鉴别、评估疗效及随访早期复发均具有重要的临床价值。Serum concentrations of CA125 were measuread with RIA in 165 samples from 98 individuals, consisting of 30 controls, 25 patients with benign ovarian tumorsand 42 with ovarian cancers, besides there was one case of junctional tumor. The results showed that CA125 levels in patients with ovarian cancers were significantly higher than those with benign tumors and controls (p<0.01). There was no significant difference of CA125 levels between benign tumor group and control group.The positive rate before surgery was 85.19% with all ovarian cancers, (The positive rate was 90% with epithelial ovarian carcinoma). CA125 levels sharply reduced after surgical operation in the patients with ovarian cancer (p<0.01). The sensitivity and specificity for detecting early recurrence of ovarian cancer were 100% and 93.75% respectively. We concluded that CA125 might be very helpful in the differential diagnosis of ovarian tumors, apprasial of the therapeutic effects and monitorign early recurrence of ovarian cancer.
