摘 要:目的了解珠江三角洲的广州、深圳、东莞、佛山4个市流动儿童口服脊髓灰质炎(脊灰)减毒活疫苗(OPV)预防接种现状及影响因素,为制定流动儿童免疫策略提供科学依据。方法对这4个市本地和外来儿童进行了脊灰抗体水平检测,应用Foxpro 5.0建立数据库,SPSS软件进行统计分析。结果共检测1180名0~6岁儿童的血标本,其中本地儿童595名,脊灰Ⅰ、Ⅱ、Ⅲ型抗体阳性率分别为99.50%、99.16%、98.15%,几何平均滴度(GMT)Ⅰ、Ⅱ、Ⅲ型分别为1:217、1:214、1:73;外来儿童585名,脊灰Ⅰ、Ⅱ、Ⅲ型抗体阳性率分别为94.53%、98.12%、94.53%,GMTⅠ、Ⅱ、Ⅲ型分别为1:145、1:193、1:58。显示外来儿童和本地儿童的脊灰抗体阳性率和GMT均在较高水平。结论广东省通过OPV常规免疫和强化免疫,已形成对脊灰病毒有效的免疫屏障。但外来儿童的脊灰抗体阳性率和GMT均低于本地儿童,因此应加强对流动人口儿童的免疫规划管理。Objective To work out a feasible preventive strategy and to evaluate the polio immune level of the floating children and related interfering factor in Guangzhou, Shenzhen,Dongwan and Fushan of Zhujiang Delta Region. Methods We detected the polio neutralizing antibody of the floating children and resident children in Guangdong. 1180 serum specimens from 0 - 6 healthy children were collected. The neutralizing antibody positive rates in resident children were 99.50% for polio Ⅰ, 99.16% for polio Ⅱ ,98.15% for polio Ⅲ ,and GMTs were 1:217,1:214,and 1:72 for polio Ⅰ, Ⅱ , Ⅲ ,respectively. The neutralizing antibody positive rates in floating children were 94.53% for polio Ⅰ, 98.12% for polio Ⅱ ,94.53% for polio Ⅲ ,and GMTs were 1:145,1:193,and 1:58 for polio Ⅰ, Ⅱ ,Ⅲ respectively. Results These results showed that both neutralizing antibody positive rates and GMTs of polio in floating children and resident children were higher. These results indicate that a firm protection barrier against poliomyelitis in Guangdong Province has been established through OPV routine immunization and supplementary immunization. But both the neutralizing antibody positive rates and GMTs of polio in floating children were rather lower than those of resident children. Conclusion The management of floating population should be strengthened.
