人端粒酶逆转录酶在喉鳞状细胞癌中表达与c-myc的相关性

摘　　要：目的研究人端粒酶逆转录酶(hTERT)mRNA在喉鳞状细胞癌中和病理分级、临床参数的关系以及和c-myc的相关性。方法用原位杂交法检测40例喉鳞状细胞癌中hTERT mRNA的表达,用免疫组织化学法检测c-myc蛋白。结果喉正常组织、不典型增生、鳞癌组织中hTERT mRNA的阳性率分别为0%(0/10)、10%(1/10)、80%(32/40),喉鳞癌中hTERT mRNA的阳性率明显高于正常组织(P<0.05)和不典型增生(P<0.05),hTERT mRNA水平与c-myc的表达呈显著相关(r=0.5422,P<0.01)。喉鳞癌中hTERT mRNA表达水平与病理分级不相关(P>0.05),与年龄、性别、TNM分期和淋巴结转移情况不相关(P>0.05)。结论喉鳞癌中hTERT mRNA的阳性率明显高于正常组织和不典型增生,有助于喉部良恶性病变的鉴别,c-myc的过度表达可能是喉鳞癌端粒酶活性升高的一个重要机制。Purpose To study the expression of human telomerase reverse transcriptase （hTERT） mRNA and its relationship with c-myc in laryngeal squamous cell cancer（LSCC）. Methods Expression of hTERT mRNA was examined by in situ hybridization; expression of c-myc was examined by immunohistochemistry in 40 cases of LSCC. Results Positive rates of hTERT mRNA in normal laryngeal tissue, laryngeal hyperplasia and LSCC were 0% （0/10）, 10%（1/10） and 80% （32/40）, respectively. The prevalence and intensity of hTERT mRNA expression were much greater in LSCC than those in normal laryngeal tissue or laryngeal hyperplasia （P〈0.05）. the positive correlation between hTERT mRNA and c-myc was significant（P〈0.01）. The expression level of hTERT were not correlated with age, gender, lymph node metastasis, TNM category and clinicopathological stage （P〉 0. 05）. Conclusions Level of hTERT mRNA was obviously higher than laryngeal normal tissues and hyperplasia. Semi-quantitative detection of hTERT mRNA expression in situ is helpful in differential diagnosis of malignant tissues and non-malignant tissues of larynx. Overexpression of c-myc may play a role in the transcriptive activation of hTERT gene in LSCC.
