牙冠延长术治疗冠根联合折断牙20例

摘　　要：目的 观察牙冠延长术治疗断缘位于龈下的冠根联合折断牙的临床效果。方法 选门诊因龋病和外伤等导致的冠根联合折断牙患者20例（23颗患牙），对其实施了牙冠延长术，并且术后6周做冠修复，观察其临床牙断端暴露效果。结果 20例23颗牙成功19颗牙，成功率82．61％，其中断缘各位点距龈缘的距离（PD）≤4mm的10颗牙有9颗牙术后断端暴露3-4mm，牙周探诊深度为2-3mm，成功率90％。4mm〈PD≤5mm的13颗牙术后6周有10颗牙断端暴露，3颗断端仍位于龈下或齐龈水平，成功率76．92％。PD≤4mm者成功率明显优于4mm〈PD≤5mm（P〈0．05）。结论 牙冠延长术治疗断缘位于龈下4mm以内，根长有1．2cm以上者残冠残根疗效确切，保持了生物学宽度，利于全冠修复，有一定的临床应用价值。Objective To observe clinical efficacy of crown lengthening surgery for the treatment of crown- root - fractured teeth,subgingival fractured edge. Methods Twenty patients （23 affected teeth） with crown - root - fractured teeth caused by caries and injury were performed with crown lengthening surgery and with restoration of crown 6 weeks after the surgery and effects of broken ends of fractured teeth were observed. Results Out of 23 affected teeth （20 cases）, 19 teeth were treated successfully and successful ratio was 82.61%. PD of 10 teeth was 4 mm and below, depth of exposure of broken ends of 10 fractured teeth was 3 - 4mm. Periodontal depth was 2 - 3mm, and successful rate was 90%. PD of 23 teeth was over 4 mm and no more than 5 ram, exposure of broken ends of fractured teeth in 10 teeth demonstrated after 6 weeks, broken ends of fractured teeth of 3 teeth were subgingival or paralleled with gum and successful rate was 76. 92%. Succeas/ul rate in patients with PD≤4mm was obviously better than patients with 4mm〈PD≤5mm （P〈0.05）. Conclusion Curative effects of crown lengthening surgery for the patients characterized by PD no more than 4 mm and residual root of residual crown was 1. 2 cm and above, which maintains biological breadth, is benefit for complete coronal restoration and is definitive value of clinical application.
