VEGF-C/Flt4系统在甲状腺乳头状癌中的表达及其与淋巴道转移关系的研究

摘　　要：目的探讨VEGF-C、Flt4在甲状腺乳头状癌中的表达及其与淋巴道转移的关系。方法用免疫组化SP法检测44例甲状腺乳头状癌及其癌旁组织VEGF-C、Flt4表达情况。结果44例甲状腺乳头状癌组织VEGF-C、Flt4阳性率分别为65.9%(29/44)和61.4%(27/44),两者阳性率在淋巴道转移组均显著高于无转移组(P<0.05),癌组织中Flt4阳性微脉管数在淋巴道转移组显著高于无转移组(P<0.05);在无淋巴结转移组,VEGF-C、Flt4的表达分别为50%(12/24)和45.8%(11/24);VEGF-C和Flt4表达情况一致的病例占总病例数的90.91%(40/44)。结论VEGF-C/Flt4系统参与了甲状腺乳头状癌淋巴道转移过程,测定肿瘤组织VEGF-C、Flt4表达及计数Flt4阳性微脉管有助于预测甲状腺乳头状癌是否伴淋巴道转移;对于无淋巴结转移而VEGF-C和Flt4呈高表达的病例,必须密切随访观察;VEGF-C可诱导并提高Flt4的表达,共同促进肿瘤的淋巴道转移。Objective To study of the relationship between expression of VEGF-C/Ylt4 and lymph node metastasis of thyroid papillary carcinoma.Methods The expression of VEGF-C/Flt4 was examined by SP immuno- histochemical technique in 44 cases of thyroid papillary carcinoma and their adjacent tissues. Results The positive rates of Flt4 and VEGF-C in thyroid papillary carcinoma are 65.9% and 61.4 % respectively. The positive rate of VEGF-C/Flt4 and the numbers of Flt4 positive microvessels, els were much higher in those with lymph node metastasis than that in those with no metastasis （P〈0.05）. The rates of VEGF-C and Flt4 are 50% （12/24） and 45.8% （11/24） respectively in the group with no metastasis. The consistent rate of expresion of VEGF-C and Flt4 is up to 90.91% （40/44） .Conclusion VEGF-C/Ylt4 may play an important role in the development and lymph node metastasis of thyroid papillary carcinomas and be useful markers to predict lymph node metastasis in the thyroid papillary carcinomas. Cases with high expression of VEGF-C and Flt4 but no metastasis must be followed up. VEGF-C can induce and enhance the expression of Flt4 and facilitate metastasis together.
