动静脉鞘膜切开术治疗视网膜分支静脉阻塞的临床研究

摘　　要：目的观察动静脉鞘膜切开术(arteriovenous sheathotomy)治疗视网膜分支静脉阻塞(BRVO)的临床疗效。方法33例病人随机分为3组,分别给予视网膜动静脉鞘膜切开术、中药、西药等治疗方法,观察患者视力、眼底彩照、眼底荧光造影(FFA)、视网膜电图(ERG)等变化。结果实验组(Ⅰ组)动静脉鞘膜切开术治疗视网膜分支静脉阻塞有效率明显高于对照2组,Ⅲ组,视力提高、静脉渗漏改善、静脉充盈时间缩短、黄斑水肿明显减退优于前2组。I、II组患者视网膜无灌注区无明显改善,III组患者视网膜无灌注区有4例患者部分改善,未发现手术并发症。结论动静脉鞘膜切开术安全、有效,疗效确切。Objective To evaluate the clinical efficacy of arteriovenous sheathotomy for the treatment of branch retinal vein occlusion （BRVO）. Methods Thirty - three patients with BRVO were divided into 3 groups and patients were treated with arteriovenous sheathotomy （ group Ⅰ）, Western medicine （ group Ⅱ ）, and Chinese medicine （ group Ⅲ ）. Postoperative examinations included color fundus photography, fundus fluorescein angiography （ FFA）, and electroretinography （ERG）. Results The effective rate of group Ⅰ was significantly higher than that of the other two groups. The vision acuity, down stream blood flow, and macular affections were significantly improved in the patients of group Ⅲ than those of the other two groups. The retinal nonperfusion regions of the groups Ⅰ and Ⅱ were not obviously improved and the retinal nonperfusion regions of 4 patients in group Ⅲ were partly improved. Conclusion Arteriovenous sheathotomy is safe and effective for the treatment of BRVO.
