摘 要:目的探讨超声造影检测肝血流动力学改变在诊断肝转移癌中的价值。方法经肘静脉团注声诺维造影剂(0.6 ml)后利用时间-强度分析软件对41例肝转移癌患者(肝转移癌组)和18例健康志愿者(正常对照组)的肝动脉到达时间、肝动脉峰值时间、肝动脉峰值强度、肝静脉到达时间、肝静脉峰值时间和肝静脉峰值强度进行测定,并计算出肝动静脉渡越时间。结果肝转移癌组的肝动脉到达时间、肝动脉峰值时间、肝静脉到达时间和肝静脉峰值时间均较正常对照组提前(P<0.01),肝动脉峰值强度和肝静脉峰值强度高于正常对照组(P<0.01),肝动静脉渡越时间较正常对照组缩短(P<0.001)。结论超声造影检测肝血流动力学改变有助于肝转移癌的诊断。Objective To investigate the value of detecting the changes of hepatic haemodynamics in diagnosis of liver metastasis by contrast enhanced ultrasound. Methods Forty-one patients with proven liver metastases( study group) and 18 normal volunteers(control group) had been recruited to be studied. Each subject was administrated with a bolus injection of the ultrasound microbubble agent SonoVue(0. 6 ml) into an arm vein. The arrival times in the hepatic artery (ATHA), time to peak in the hepatic artery (TTPHA), peak intensity of the hepatic artery(PIHA ), arrival times in the hepatic vein(ATHv ), time to peak in the hepatic vein(TTPHv) and peak intensity of the hepatic vein(PIHv ) were measured with the use of time-intensity curve software. The hepatic artery to vein transit time(HAVTT) was calculated as the difference between the arrival times in the hepatic artery and the hepatic vein. Results ATHA, TTPHA, ATHv and TTPHv in study group were significantly shorter than those of control group( P 〈0. 01 ). PIHA and PIHv in study group were significantly brighter than those of control group( P 〈0.01). HAVTT was significantly in study group shorter than that of control group( P 〈0. 001 ). Conclusions Detecting the changes of hepatic haemodynamics by contrast enhanced ultrasound help to diagnose liver metastases.
