摘  要:目的 应用细胞图像分析术(ICM—DNA)对大宗病例非霍奇金淋巴瘤(NHL)进行DNA参数的检测,寻找对NHL的诊断、鏊别诊断和预后有指导意义的参考指标。方法 对210例NHL(WH02001新分类)病例及30例良性增生淋巴组织(RH)行改良的Feulgen染色并作ICM-DNA检测。结果 NHL平均DNA含量为7.01~10.77Pg;SPF为10.97%~30.87%,异倍体率7.76%~17.76%,均高于对照组(P≤0.01);NHL二倍体百分含量为16.03%~70.90%,低于对照组(P≤0.01);NHL侵袭组、高侵袭组的平均DNA含量、SPF、异倍体率均高于惰性组(p≤0,01);FL—Ⅱ、Ⅲ级的平均DNA含量、SPF、异倍体率均高于FL-Ⅰ级(p≤0.01)。结论 ICM—DNA检测中,平均DNA含量、SPF、异倍体率、二倍体百分含量对良恶淋巴组织增生的鉴别诊断、NHL惰性或侵袭性预后判断均为有意义的参考指标;但对侵袭性高低无提示,FL是相对异质化群体,不能简单归为惰性。Objective To investigate the dignositic and prognositic significance of DNA parameters by DNA image cytometry (ICM-DNA) analysis in the non-Hodgkin' s lymphoma (NHL). Methods ICM- DNA was performed on 210 NHL and 30 RH specimens prepared by Feulgen stain. DNA content, Diploid fraction, Aneuploid fraction, and S-phase fraction (SPF) were computed. Result DNA content (7.01~ 10.77 pg), SPF( 10.97 ~ 30.8), and Aneuploid fraction (7.76% ~ 17.76% ) in NHL was higher than in RH(p≤0.01 ) ; but Diploid fraction (16.03% ~70.90% ).in NHL was lower. There is significant difference between indolent and aggressive, indolent and highly aggressive, FL-Ⅰ and FL-Ⅱ, FL-Ⅰ and FL-Ⅲ (P≤0.01), but nonsignificant difference between aggressive and highly aggressive, FL-Ⅱ and FL-Ⅲ in all parameters investigated (P 〉 0.05 ). Conclusion DNA content, Diploid fraction, Aneuploid fraction, and SPF can be useful in distinguishing between malignant and benign lymphoid hyperplasias, indolent and aggressive/highly aggressive, but none of them could differentiates between aggressive and highly aggressive; FL should not be simply taken as indolent NHL.
