直肠癌手术并发症的原因分析及处理对策

摘　　要：目的：探讨直肠癌手术并发症发生的原因、预防措施及处理方法。方法：回顾性总结分析1990-2004年间10例直肠癌手术并发症的临床资料。Miles手术7例，直肠前切除3例；并发症包括吻合口漏1例，尿道损伤2例，盆底腹膜裂孔疝2例，骶前静脉丛出血2例，前列腺出血1例，性功能及排尿功能障碍各1例。结果：盆底腹膜裂孔疝、吻合口漏经再次手术痊愈，尿道损伤采用修补及吻合痊愈，骶前静脉丛出血用ZT胶粘堵，图钉法控制出血，前列腺出血用腹会阴联合捆扎控制出血。结论：直肠癌手术并发症重在预防，及时有效地处理这些并发症也很关键。Objective：To investigate the possible causes of complication , prevention and management measures in surgery of rectal cancer. Methods： A restrospective study of 10 patients with complications in surgery of rectal cancer from 1990-2004 was performed. Seven patients were underwent Miles surgery and anterior rectum resection was operated in 3 patients; Complications included anastomosis leakage in one patient, urethral injury in two,diaphragma pelvis-periteneal hiatus hernia in two, presacral veniplex bleeding in two,and prostate bleeding in one,paraeccrisis and hypaphrodisia in one. Results： Diaphhragma pelvis-periteneal hiatus hernia, anastomosis leakage was cured after second surgery , urethral injury were cured after urethral neoplasty or anastomosis, ZT gum and pressing thumb pin were employed to control presacral venous bleeding, abdominoperineal symphysial ligation was employed to control prostate bleeding. Conclusion： It is important to prevent complications in surgery of rectal cancer and management of the complications in time and effectively is key as well.
