摘 要:目的比较失血性休克家兔使用生理盐水、6%羟乙基淀粉液(贺斯)、高渗高胶液复苏的效果。方法将30只家兔随机分为生理盐水组(NS组)、6%羟乙基淀粉液组(HAES组)、高渗高胶液组(HHS组),3组均自股动脉匀速放血至平均动脉压40 mmHg,稳定30 min后,分别输注相应的液体。记录休克前及治疗后10,60,120,180 min各时点平均动脉压(MAP)、左室舒张末压(LVEDP)、左室内压最大收缩率(+dp/dt max)、血钠及尿量。结果3种液体均能在休克治疗初期达到相似的血流动力学效果,治疗120 min后,HHS组的MAP、LVEDP、+dp/dtmax比其余2组高(P<0.05)。结论高渗高胶液维持血流动力学平稳时间较长,是治疗失血性休克的理想液体。Objective It is to compare the resuscitative effects of normal saline (NS), 6% hydroxyethyl starch (HAES) and hypertonic-hyperoneotie solution (HHS) in rabbits with hemorrhagic shock. Methods 30 rabbits were randomly divided into NS group, HAES group and HHS group. All the rabbits were exanguinated from femoral artery with a steady speed until mean arterial pressure lowered to 40 mmHg. After 30 minutes' waiting, the three groups were given NS, HAES and HHS respectively. MAP, LVEDP, + dp/dt max, serum sodium and urine output were recorded before shock and after 10, 60, 120, 180 minutes' resuscitation. Results All the three kinds of liquid got similar effect of hemodynamies during the first period of resuscitation. After 120 minutes' treatment, MAP, LVEDP, + dp/dt max were higher in HHS group than those in the other two groups (P 〈 0.05). Conclusion HHS can maintain hemodynamies steadily for longer time, so it is an effective kind of treatment fluid for hemorrhagic shock.
