p53和Rb与甲状腺乳头状癌分化程度关系的研究

摘　　要：目的:通过检测p53和Rb蛋白在不同分化程度甲状腺乳头状癌中的表达,探讨其在不同分化程度甲状腺乳头状癌发生、发展中的意义。方法:采用免疫组化LSAB法,对62例甲状腺乳头状癌术后原发灶标本中p53及Rb蛋白表达情况进行检测,按显微镜下甲状腺乳头状癌细胞的组织学特点,分为Ⅰ型(分化较好型)、Ⅱ型(中间型)、Ⅲ型(分化较差型)三型。结果:1)三型不同分化程度甲状腺乳头状癌中,癌细胞分化程度越好,p53的阳性表达率越低,而Rb的阳性表达率越高,p53阳性表达率差异有统计学意义,P=0.005,Rb阳性表达率差异也有统计学意义,P=0.001。2)在甲状腺乳头状癌中p53和Rb的蛋白表达无相关性,P=1。结论:p53蛋白的高表达与Rb蛋白的低表达在甲状腺乳头状癌向不同分化方向发展方面可能起重要作用,甲状腺乳头状癌细胞分化程度越差,p53的阳性表达率越高,而Rb的阳性表达率越低,但两者并无关联性。OBJECTIVE： To explore the expressions of p53 and Rb in papillary thyroid carcinoma （PTC） with different differentiotions, and to find out their signifieance in PTC. METHODS： Sixtytwo cases of original papillary thyroid carcinoma were retrospectively reviewed. The expressions of suppressor gene p53 and Rb were examined by the immunohistochemical LASB staining method in each postoperative primary site. According to the histological characteristics of PTC under a microscope, PTC was divided into three groups： type （well-differentiated PTC）, type Ⅱ （moderate-differentiated PTC） and type Ⅲ（poor-differentiated PTC）. RESULTS： 1）The expression of p53 was positively related to the differentiation and the expression of Rb was negtively related to the differentiation. The different positive expression rates of p53 in three types of PTC were significant, P= 0.005. The different positive expression rates of Rb in three types of PTC were significant, P=0.001. 2）The expression of p53 was not associated with the expression of Rb in PTC, P=1. CONCLUSION： Both over expression of p53 and descended expression of Rb may play an important role in the tumorigenesis and development of tumor in the differentiation of PTC. The expressions of suppressor gene p53 and Rb are correlated with the cytodifferentiation of PTC. The suppressor gene p53 were not related to Rb in the course.
