不同证型2型糖尿病患者与听力变化的相关性

摘　　要：目的：分析不同证型2型糖尿病患者的听力变化特征，为有效治疗耳聋提供依据。方法：选择2004年6月-2005年6月来山东潍坊医学院附属医院就诊的2型糖尿病患者56例，年龄在40-65岁，按中医辨证分型，并进行听力测试，测试结果与本院健康体检者进行对照分析。结果：56例2型糖尿病患者，中医辨证分型为肺热津伤型（10例），胃热炽盛型（16例），阴阳两虚型（17例），肾阴亏虚型（12例）。纯音测试结果，各组证型与健康对照组相比有显著性差异（P〈0．05，0．01），肾阴亏虚型和阴阳两虚型，与其他证型相比有显著性差异（P〈0．05），2型糖尿病患者听力测试各频段均有变化，以高频听力下降为主与中、低频段相比（P〈0．05）。结论：不同证型2型糖尿病是听力下降或耳聋的主要因素。听力下降以高频为主，辨证施治为其理论依据。Objective： To analyze the characters of the hearing changes in type 2 diabetic patients of different syndrome types ,so as to provide evidence for the effective treatment of deafness. Methods： 56 type 2 diabetic patients with deafness aged 40 - 65 years, who were treated in the Affiliated Hospital of Weifang Medical College between June 2004 and June 2005, were typed according to tradi- tional Chinese medicine （ TCM ） syndrome differentiation, and they also participated in the hearing test,and then the results were compared with those of the healthy physical examinees in this hospital. Results：Of the 56 type 2 diabetic patients,the TCM syndrome differentiation was body fluid impairment due to lung heat in 10 cases, excessiveness of stomach heat in 16 cases, deficiency of both yin and yang in 17 cases ,deficiency of kidney - yin in 12 cases. The resuits of hearing test in the syndrome type groups were significantly different from that in the healthy control group（ P 〈0.05 - 0.01 ）, those in the patients of the deficiency of kidney-yin type and deficiency of both yin and yang type were significantly different from those in the patients of the other types. The hearing of the type 2 diabetic patients had changes at all the frequencies, and mainly the decrease of hearing at high frequency as compared with middle and low frequencies （P 〈 0.05）. Conclusion： Type 2 diabetes mellitus is the main factor for the hearing decrease or deafness, and most hearing decrease is at high frequency, the theoretical evidence is the diagnosis and treatment based on the syndrome differentiation.
