胃癌分化程度、淋巴结转移和Hp感染与Bcl-2基因表达的关系

摘　　要：目的了解胃癌的分化程度、淋巴结转移和Hp(幽门螺杆菌)感染与Bcl-2基因表达的关系,探讨Hp在胃癌发生、发展过程中的机制。方法应用免疫组化和Warthin-Starry染色方法对76例胃癌组织中Bcl-2基因蛋白表达情况和幽门螺杆菌感染情况进行检测。结果无淋巴结转移24例,Bcl-2阳性14例,有淋巴结转移52例,Bcl-2阳性35例,两者之间差异无显著性(P>0.05)。分化型腺癌23例,Bcl-2阳性17例,低分化腺癌53例,Bcl-2阳性25例,两者之间差异有显著性(P<0.05)。46例Hp阳性病例中,Bcl-2阳性36例,30例Hp阴性病例中,Bcl-2阳性16例,两者之间差异有显著性(P<0.05)。结论Bcl-2基因表达与胃癌淋巴结转移无关,与胃癌的分化程度和Hp感染有关。分化型胃癌Bcl-2基因表达高于低分化型胃癌,Hp阳性胃癌患者Bcl-2基因表达高于Hp阴性患者。Hp可能为促癌剂在胃癌的发生发展中起一定的作用。Objective Understand the relation between the degree of differentiation of gastric carcinoma, lymph nodes status, Helicobacter pylori （Hp） infection and Bcl-2 gene expression, probe into the mechanism in the course of taking place, developing in gastric carcinoma of Hp.Method Bcl-2 was examined by immunohistochemically, and Hp examined by Warthin-Starry method in 76 with gastric carcinoma. Result： There is not metastatic carcinoma in lymph nodes 24, Bcl-2 is positive 14, there is metastatic carcinoma 52, Bcl- 2 is positive 35, there is no significance in the difference between the two （P〉0.05）. Well-differentiated adenocarcinoma 23, Bcl-2 is positive 17, poorly-differentiated adenocarcinoma 53, Bcl-2 is positive 25, and there is significance in the difference between the two （P 〈0. 05）. In 46 Hp positive case, Bcl-2 is positive 36, in 30 Hp negative case, Bcl-2 is positive 16, there is significance in the difference between the two （P〈0.05）. Conclusion Bcl-2 gene expression has nothing to do whether there is metastatic carcinoma in lymph nodes, relate to the fact that the degree of differentiation and Hp of the gastric carcinoma are infected. Well-differentiated adenocarcinoma Bcl-2 gene expression is higher than the poorly-differentiated adenocarcinoma, Hppositive stomach cancer patient Bcl-2 gene expression is higher than the negative patient of Hp. Hp might act as a tumor promoter in the genesis of gastric cancer.
