术前超选择保留动脉导管持续动脉化疗在宫颈癌治疗中的临床价值

摘　　要：目的探讨宫颈癌术前超选择保留动脉导管持续动脉化疗的临床疗效。方法对56例宫颈癌患者(Ⅰb2期8例,Ⅱa期16例,Ⅱb期32例)行术前超选择保留动脉导管持续动脉化疗,其中4例行化疗栓塞,评定其疗效。结论总有效率(CR+PR)为67.86%。肿瘤直径≤5cm、>5cm的患者的有效率分别为81.25%、50.00%,两者比较有显著性差异(P<0.05);不同组织类型鳞癌、腺癌、腺鳞癌间比较无差异(P>0.05)。经1～2疗程,56例患者顺利行宫颈癌广泛子宫切除术+盆腔淋巴结清扫,4例病理检查无肿瘤残留,2例有宫旁浸润,3例淋巴结转移阳性,1例腺管癌栓。结论术前超选择保留动脉导管持续动脉化疗能使肿瘤体积缩小,控制宫旁淋巴结转移,提高手术切除率,增加宫颈癌的治愈率。OBJECTIVE Probe into the clinical curative effect that kept artery pipe lastingartery chemotherapy before the cervical carcinoma skill. METHODES Treat 28 subjects （4 of stage Ⅰb2, 8 of stage Ⅱa,16 of stage Ⅱb） with kept artery pipe lasting artery chemotherapy before the skill for cervical carcinoma, among which 4 were treated with chemotherapy and embolization, and evaluate the effectiveness. RESULTS The general rate of effectiveness is 67.86%. The rates of lettectiveness of subjects with tumor diameter ≤ 5cm, 〉5cm are respectively 81.25%, 50.00%, whichhad remarkable difference （P〈0.05）, there are no difference between different Organize type, squamouscarcinoma, adenocarcinoma and adeno-squamouscarcinoma. （P〉0.05） . After 1 -2 therapyprocesses, 28 subjects were successfully treated with radical surgery for uterine and pelvic lymph node, among which 4 had no left of tumor through pathological check, 2 had invasion around theuterine, 3 were positive of lymph transfer, 1 had adenocarcinoma embolization. CONCLUSIONS Exceed and choose to keep artery pipe lasting artery chemotherapy for cervical carcinoma cancontract the tumor, control the transfer of lymph around uterine, improve the successful rate of operational cut and the rate of cure, so have broad clinical prospects.
