摘 要:目的研究Toll样受体(TLR)4(896A>G)在急性胰腺炎(AP)患者和正常人群中的分布及其与AP的相关性。方法采用错配PCR-RFLP分析方法检测AP患者和健康自愿者人群TLR4(896A>G)等位基因频率。结果(1)所有TLR4(896A>G)的基因突变均为杂合子;(2)与健康志愿者组(5.6%)相比,轻型急性胰腺炎(MAP)组TLR4G等位基因频率(9.8%)呈轻度升高(差异无显著性,P=0.182);(3)与健康志愿者组(5.6%)相比,胰腺组织坏死+感染的重型急性胰腺炎(SAP)组TLR4G等位基因频率(20%)明显升高(差异有显著性,P=0.019)。结论TLR4错义突变(896A>G)在AP的发生发展过程中具有双刃性作用:(1)AP早期为一种保护因素;(2)AP后期表现为一种危险因素,TLR4(896A>G)的单核苷酸多态性可能与SAP存在相关性。Objective To study the relationship between Toll- like receptor 4 ( 896A 〉 G ) polymorphisms and acute pancreatitis. Methods TLR4( 896A 〉 G )mutation was assayed by mispaifing PCR - RFLP analysis in AP pantents and healthy volunteers. Results (1) All individuals with Toll - like receptor 4 mutations were heterozygous for the TLR4 ( 896A 〉 G ). (2)Compared with healthy volunteers (5.6%), the G allele frequency of the TLR4 increased slightly in patients with mild acute pancreatitis (9.8%) without significant difference ( P = 0.182). (3)Compared with healthy volunteers (5.6%), significant difference was found in infection of pancreatic necrosis (20%) in severe AP patients ( P =0.019).Conclusions Human TLR4 ( 896A 〉 G ) mutations have the bidirectional role in the pathogenesis of AP, i. e. protective function in the early stage of AP and risk factor in the later stage of AP.
