摘  要:目的:探讨肿瘤抑制基因P16INK4A、凋亡相关基因异构体ΔNP63和细胞转录因子Brn-3a在各级宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)的TCT标本中的表达及其作为CIN生物标志物的可行性和临床意义。方法:利用SP免疫组化法检测71例TCT标本中P16INK4A和ΔNP63的表达;用RT-PCR法检测各标本中Brn-3a mRNA的转录,分析它们与CIN病理分级的关系。结果:各级病理学诊断中P16INK4A与ΔNP63阳性率分别为炎症(4.76%/4.76%),CINⅠ(89.47%/84.21%),CINⅡ(100%/100%),CINⅢ(100%/100%),各级CIN与炎症的P16INK4A和ΔNP63阳性率之间,差异有极显著性(P<0.001),各级CIN的阳性率之间,差异无统计学意义(P>0.05)。Brn-3a的表达阳性率分别为炎症(0%),CINⅠ(5.26%),CINⅡ(50%),CINⅢ(90.48%),CINⅢ的Brn-3a阳性率明显高于与炎症、CINⅠ组(P<0.01),炎症与CINⅠ、CINⅡ与CINⅢ比较,差异无统计学意义(P>0.05)。结论:P16INK4A、P63与Brn-3a可作为CIN的标志物,辅助细胞学筛查,Brn-3a可能成为间接反映CIN发展的生物学指标。Objective :To explore the potential of P16^INK4A △NP63 and Brn-3a as biomarkers for cervical premalignant lesions in Thin Prep smears and to search for the identification of specific biomarkers for cervical intraepithelial neoplasia(CIN) as a new way to prejudge the progress of CIN. Method: Using streptavidin peroxidase (SP) method, immunocytochemical analysis of P16^INK4A and △ANP63 expression was performed on 71 Thin Prep smears. RT-PCR was used to detect the mRNA of Brn-3a in these liquid-based smear specimens. Results: Both positive of P16^INK4A and △NP63 in CIN was higher than that in inflammation(P 〈0.01 ). Both positive of CINⅡ and Ⅲ about Brn-3a was higher than that of inflammation(P 〈0.01 ). Conclusion:Overexpression of P16^INK4A and △NP63 can be used as biomarkers to indicate CIN. Brn-3a can be another useful biomarker to indicate CIN,which shows indirectly the development of CIN.
