羊膜移植联合结膜角膜移植治疗严重眼化学伤

摘　　要：目的探讨羊膜移植联合结膜角膜移植对严重眼部化学烧伤的临床治疗效果。方法对严重眼部化学烧伤患者26例（32眼）采用羊膜移植联合结膜角膜移植，术后随访2～14月（平均6月）。结果术后3～7天，眼部炎症控制，羊膜移植片未见免疫排斥反应或融解吸收，24眼角膜恢复透明，5眼角膜白斑，3眼全角膜浑浊，全部患者保存了眼球，眼表稳定，角膜和结膜融解未继续进展。结论人羊膜移植可以有效地促进上皮细胞分化移行，抑制局部的炎症反应，阻止新生血管和瘢痕形成，阻止烧伤的病理发展。Objective To investigate the therapentic effect of human amniotic membrane on conjunctival and corneal transplantation in severe ocular chemical burn. Methods Twenty-six cases （32 eyes） of severe ocular chemical burns were treated with human amniotic membrane combined with conjunctival and corneal transplantation. After operation 2- 14 months （ mean 6 months ） were followed up. Results Ocular inflammation was controlled in 3 - 7 days after surgery. No immunolcgical rejection, necrosis and graft absorption were found. There were corneal transparency in 24 eyes, corneal scaring in 5 eyes and opacity in 3 eyes. All eye balls were saved and had stable ocular surface. No furgher progress of the lysis of cornea and conjunctiva was found after operation. Conclusions Human amniotic membrane can effectively advance the polarization and migration of epithelium cell, reduce local inflammation reaction, neovascularization and fibrosis, stop the burned pathologic process.
