特高龄晶状体超声乳化的安全性及效果

摘　　要：目的探讨特高龄晶状体超声乳化人工晶状体植入术的安全性及效果。方法在术前准备充分和备应急防范措施下，进行38例（43眼）90～106岁特高龄老年性晶状体超声乳化人工晶状体植入术，随机抽取同期相同方法手术80-90岁40例（46眼）白内障为对照组。结果生命安全性：术中特高龄组8例（21．05％），对照组2例（5．00％）出现生命征异常需相应治疗。术后3月矫正视力：特高龄组〈0．5者10眼（23．25％），≥0．5者28眼（65．12％），≥1．0者5眼（11．63％）；对照组〈0．5者7眼（15．22％）、≥0．5者32眼（69．56％）、≥1．0者7眼（15．22％），两组比较差异有显著性（P〈0．05）。术中并发症：特高龄组后囊破裂5眼（11．63％）、对照组2眼（4．35％）。结论特高龄患者手术确有风险，在术前准备充分，术中应急措施完善，由操作娴熟有一定技巧的术者短时间完成手术，特高龄患者手术可取得视力提高、生命安全的效果。Objective To investigate the efficiency and safety of phacoemulsification and intraocular lens implantation in the patients of age over 90. Methods 38 cases （43 eyes） age over 90 and 40 cases （46 eyes） from age 80 to 90 as the control group （randomly） were treated by phacoemulsification. Its safety, recovery and complications were reviewed during and after the operation. Results 7 cases （ 18.42% ） in the former group and 2 cases （5%） in the latter group were abnormal in vital signs, necessary for the use of treatment. After 3 months, corrected visual acuity was 〈 0.5 in 10 eyes （23.26%）, 90.5 in 28 eyes （65.12%） and ≥1.0 in 5 eyes （11.62%） in the former. And 〈0.5 was in 7 eyes （15.22%） in the latter.There was a significant difference between the two groups. Conclusion The preparation and management before and during the operation are the key point for visual acuity recovery and surgery be safety and efficiency.
