川芎嗪对眼压控制下原发性开角型青光眼患者视功能和视网膜血循环的影响

摘　　要：目的探讨川芎嗪对控制在目标眼压范围的原发性开角型青光眼患者视功能的保护作用。方法治疗组用川芎嗪注射液静脉点滴,对照组口服尼莫地平片,两组各30例。治疗前后观察视力、眼压、眼底和视野、共焦激光视网膜断层扫描、共焦激光视网膜多普勒血流等检查。结果两组治疗前后的视力改善均无统计学意义(P>0.05)。治疗组治疗前后视野平均光敏感度(P<0.05)、视野平均缺损(P<0.05)和视野丢失方差(P<0.05)的改善均有统计学意义。对照组对视野平均光敏感度(P<0.05)的改善有统计学意义;视野平均缺损和视野丢失方差的改善没有统计学意义。治疗组对视盘筛板区血管容量(P<0.05)的改善有统计学意义,对照组(P>0.05)对视盘筛板区血管容量的改善没有统计学意义。治疗组(P>0.05)与对照组(P>0.05)治疗前后对视盘筛板区血流量的改善没有统计学意义。治疗组(P<0.05)与对照组(P<0.05)治疗前后对视盘筛板区红细胞移动速率的改善均有统计学意义。结论川芎嗪可以改善眼压控制后青光眼患者的视野平均敏感度、视野平均缺损和视野丢失方差。尼莫地平可以改善眼压控制后青光眼患者的视野平均敏感度。川芎嗪可以改善眼压控制后青光眼患者的视盘筛板区血管容量。川芎嗪和尼莫地平可以改善眼压控制后青光眼患者的视盘筛板区红细胞移动速率。川芎嗪和尼莫地平对青光眼患者的视功能保护作用尚待进行多中心和大样本的临床研究。OBJECTIVE To explore the protecting role ; intraocular pressure; visual function of ligustrazine protecting visual function with primary open - angle glaucoma （POAG） . Their intraocular pressure has been controlled. METHODS the patients There were 30 patients with POAG under controlling intraocular pressure in treatment group and control group respectively. Ligustrazine was injected by vein in treatment group. Nimodipine was taken orally in control group. Visual acuity, intraocular pressure, fundus and visual field were examined before and after treatment. RESULTS There was no statistical significance for promotion of visual acuity before and after treatment in treatment group and control group （ P 〉 0.05） . There was statistical significance in increasing mean sensitivity （ P 〈 0.05）, mean defect （ P 〈 0.05）, loss variance （P 〈 0.05） of visual field before and after treatment in treatment group.There was statistical significance in increasing mean sensitivity of visual field before and after treatment in control group （ P 〈 0.05） . CONCLUSIONS Ligustrazine was able to increase mean sensitivity, mean defect, and loss variance of visual field in the patients with glaucoma controlling intraocular pressure. Nimodipine was able to increase mean sensitivity of visual field of glaucoma patients under controlling intraocular pressure.The role of ligustrazine and nimodipine protecting visual function of patients with glaucoma needs to be studied through multicenter and a large sample further.
