抗原处理相关转运蛋白和星形细胞瘤的相关性研究

摘　　要：目的探讨星形细胞瘤中抗原处理相关转运(TAP)蛋白分子表达及其临床意义,探讨星形细胞瘤的治疗策略。方法采用免疫组织化学技术对50例星形细胞瘤患者肿瘤组织和15例来自于脑外伤内减压手术、高血压脑出血及脑血管畸形手术中所取的正常脑组织中TAP蛋白的表达进行了检测。结果肿瘤组与正常组TAP蛋白阳性细胞百分率比较有显著性差异；低级别星形细胞瘤25例与高级别星形细胞瘤25例TAP蛋白平均阳性细胞百分率比较有显著性差异；低级别星形细胞瘤与高级别星形细胞瘤病例TAP蛋白表达阳性率比较有显著性差异；50例星形细胞瘤患者中复发者与未复发者TAP蛋白表达阳性率比较有显著性差异：死亡者与未死亡者TAP蛋白表达阳性率比较有显著性差异；50例星形细胞瘤患者中TAP蛋白表达阳性者与表达阴性者GOS评分比较有显著性差异。结论星形细胞瘤患者存在TAP蛋白分子低表达,推测其促使星形细胞瘤患者肿瘤细胞逃避免疫监视；推测TAP蛋白缺失率可成为预测星形细胞瘤的发生、恶性程度及其预后的指标之一。Objective To detect the expression of transporter associated with antigen processing （TAP） protein and discuss the correlation between TAP protein and pathological grade, rate ofrelaps and prognosis of astrocytoma, investigate the value of TAP in evaluating malignancy and predicting prognosis. Methods All brain tissue samples from 50 astrocytoma cases （tumor group） and 15 cases （normal group） of cerebral trauma with intracranial decompression, or hypertensive intracerebral hemorrhage or cerebral vascular malformation were hydrated through graded alcohols, then the antigen were repaired by microwave, and the expression of TAP protein were determined by S-P immunohistochemical analysis. Results Among the 50 cases with astrocytoma, 27 cases were positive in the TAP protein expression and the percentage of TAP protein was 0.291 ±0.2724; among the 15 normal cases, all were positively expressed and the percentage of TAP protein was 0.584±0.2169; there was significant difference between the tumor group and the normal group（P〈0.05）. The percentage of TAP protein was 0.374±0.2913 in 25 cases with low-grade （ Ⅰ or Ⅱ ） astrocytoma, and it was 0.228±0.2008 in 25 cases with high-grade （Ⅲ or Ⅳ） astrocytoma; significant difference was also found between them （P〈0.05）. What＇s more, 17 cases were positively expressed in 25 cases with low-grade astrocytoma, and 10 cases were positively expressed in 25 cases with high-grade astrocytoma; the positive rate was significantly different between the 2 groups （P〈0.05）. Among the 50 cases with astrocytoma, 10 cases were positively expressed in 27 relapse cases, and 17 cases were positively expressed in 23 non-relapse cases; the difference was significant in positive rate between the relapse and non-relapse cases （P〈0.05）. Among the 50 cases, 6 cases were positively expressed in 21 dead cases, and 21 cases were positively expressed in 29 survival cases; the difference was significant（P〈0.05）.
