摘  要:目的研究碘染色对食管早癌和不典型增生的检出率,阐明碘染色的临床意义。方法对385名行内镜检查的食管癌高危人群进行2.5%卢戈氏碘液染色,在直径>0.5cm的不染区或淡染区内取活检,计算轻、中、重度不典型增生和早癌的检出率。结果轻度、中度和重度不典型增生/早癌的检出率分别为7.5%、2.6%和3.4%。碘染色对诊断重度不典型增生的敏感性、特异性、阳性预测值和阴性预测值分别为84.6%、92.5%、40.7%和99.0%。结论内镜下碘染色能提高食管早癌和不典型增生的检出率,是诊断食管早癌和不典型增生的一种安全、简便而有效的方法。Objective To study the detection rate of dysplasia and early esophageal squamous cell carcinoma (ESCC) using Lugol' s chromoendoscopy. Methods Three hundred and eighty-eight patients who received routine gastroscopy and had high risk of ESCC were examined by the iodine dyeing on their esophageal mucosa with 2.5% Lugol' s solution. Multiple biopsies were taken from the unstained or light-stained areas when their diameters were over 0.5cm for pathology. Results With the Lugol' s staining method,the incidences of light,moderate and high grade dysplasia/early ESCC were 7.5% (29/ 385 ) ,2.6% ( 10/385 ) and 3.4% (13/385) respectively. Lugol' s chromoendoscopy showed a sensitivity of 84.6% ,a specificity of 92.5% ,a positive predictive value of 90.7% and a negative predictive value of 99.0% to diagnose early ESCC/dysplasia. Conclusion Lugol' s chromoendoscopy may improve the diagnosis of early ESCC/dysplasia.
