前方牵引联合快速扩弓矫治骨性Ⅲ类错的临床研究

摘　　要：目的探讨前方牵引联合快速扩弓对替牙期骨性安氏Ⅲ类错中颌骨和牙齿的作用和影响。方法对10例安氏Ⅲ类错替牙晚期患者使用前方牵引联合快速扩弓治疗,并对治疗前、后的头颅侧位片进行X线头影测量分析。结果治疗后SNA、ANB、NA/PA、MP/SN、U1/SN、V-A、Ptm'-A'及前下面高显著增加,而SNB、PP/SN、L1/MP显著减小,C0-G0-Gn、Ar-Pg、G0-Pg无显著性变化。结论上颌前牵引治疗后上颌基骨明显前移并向前生长,同时伴有腭平面的逆时针旋转;下颌骨向下向后旋转,其生长暂时受到抑制;上切牙唇倾,下切牙舌倾起到代偿性掩饰作用。Objective To study skeletal and dental changes in correction of skeletal class Ⅲ malocclusion by protraction headgear combined with rapid palatal expansion（RPE） in the mixed dentition. Methods 10 patients with skeletal anterior crossbite were selected and treated with protraction headgear combined with rapid palatal expansion in the mixed dentition. Lateral cephalometric roentgenograms were taken before and after treatment, and were analysed to evaluate the changes. Results After treatment, SNA, ANB, NA/PA, MP/SN, U1/SN, V-A, Ptm‘-A＇ and ANS＇-Me increased significantly, and SNB, PP/SN, L1/MP decreased significantly, but C0-G0-Gn、 Ar-Pg and G0-Pg have no significant change. Conclusion With protraction the maxillary move and grow forward accompanied with counterclockwise rotation of palatal plane, and the mandible rotate backward and downward and its growth were restrained temporarily. In addition, the upper incisor incline labially and the lower incisor incline lingually, which can help to conceal the deficiency.
