基托粘结式上颌前方牵引器矫治乳牙期骨性反的疗效观察

摘　　要：目的观察基托粘结式上颌前方牵引器矫治乳牙期骨性前牙反的临床疗效。方法选取20例乳牙期骨性前牙反的临床病例,采用基托粘结式上颌前方牵引器进行矫治,矫治前后均拍摄头颅X线侧位片,进行头影测量分析,并对测量结果进行统计学分析。结果矫治后SNA增加(2.3±1.1)°,SNB减小(1.1±0.9)°,ANB增大(3.5±2.2)°,前牙反全部解除;面凸角增加(4.3±2.1)°,全面凸角减小(4.8±2.6)°,患者侧貌由Ⅲ类凹面型变为直面型。结论基托粘结式上颌前方牵引器可以有效地矫治乳牙期骨性前牙反,调整上、下颌骨间前后向的位置关系,改善患者软组织侧貌。Objective To evaluate the effects of acrylic bonded protraction appliance in correcting primary skeletal crossbite. Methods Twenty children with skeletal crossbite were selected and treated with acrylic bonded protraction appliance. Cephalometric radiographs were taken and analyzed before （T1） and after treatment （3＂2）. Students＇t-test was used to determine if there were significant differences between the 2 time points. Results SNA increased by （2.3 ± 1. 1 ） °, SNB decreased by （ 1.1 ± 0.9） °, and ANB increased by （ 3.5 ± 2.2） °. Ns-Sn-Pos increased by （4.3 ：±2. 1 ） °, Ns- Prm-Pos decreased by （4. 8 ± 2. 6 ）°. All of the anterior crossbites were corrected and the profiles were improved satisfactorily. Conclusion Acrylic bonded protraction appliance is a clinically effective method for correcting primary skeletal crossbite,balancing anteroposterior jaw relationship and improving the face profile.
