摘 要:目的应用荧光定量RT-PCR(Fluorogenic quantitative reverse transcriptase polymerase chain reaction,RT-PCR)检测肺癌外周血中微转移情况。方法用荧光定量RT-PGR方法检测103例肺癌患者外周血CK19mRNA和CK20mRNA表达。结果103例肺癌患者外周血中CK19mRNA检测阳性35例,阳性率33.98%;CK20mRNA检测阳性17例,阳性率16.50%,其中6例患者CK19、CK20mRNA均阳性。统计结果显示外周血CK19、CK20mRNA的表达与临床分期、分化程度和淋巴结转移相关,与性别、年龄、病理类别无关。结论用荧光定量RT-PCR法检测肺癌患者外周血CK19mRNA和CK20mRNA具有较高的灵敏度和特异性,有助于早期筛选亚临床转移。Objective To detect the micrometastases status in peripheral blood of cancer patients by fluorogenic quantitative reverse transeripition - polymerase chain reaction( RT - PCR). Methods CK19mRNA and CK20mRNA expression in peripheral blood was detected in 103 cases of lung cancer by fluorogenic quantitative RT - PCR. Results Positive CK19mRNA and CK20mRNA was detected in peripheral blood in 35 and 17 of 103 patients with lung cancer respectively. The positive rate were 33.98% and 16.50% respectively, and among them ,there were 6 cases in which CK19,CK20mRNA were both positive .Statistical analysis indicated that expression of CK19mRNA in peripheral blood was not correlated with gender, age and pathological type of the cancer, but was correlated with clinical TNM stage, lymph node metastasis status and cell differentiation. Conclusion RT - PCR amplification of CK19mRNA and CK20mRNA is a sensitive and special method to detect early haematogenons discrimination of cancer cells for patient with lung cancer, and may lead to early diagnosis of patients with sub clinical metastasis in circulation.
