摘  要:目的评价一种新的羊膜缝合方法——连续交锁缝合,在眼表羊膜移植术中应用及其临床疗效。方法回顾总结我院行羊膜移植术的眼表疾病患者57例(65眼),观察组26例(31眼)行连续交锁缝合,与对照组31例(34眼)行间断或单纯连续缝合者进行比较,随访时间均超过2m,观察两组羊膜脱落时间、创面上皮化时间等指标。结果两组3w内羊膜自行脱落眼数比:观察组3/31眼明显低于对照组为15/34眼,两者之间的差别有统计学意义(P=0.018);羊膜完整存留3w以上眼数比:观察组19/22眼高于对照组6/21眼,两者之间的差别有统计学意义(P=0.044);两组2w内角膜上皮化眼数比:观察组17/28眼高于对照组12/32眼,但两者之间的差别无统计学意义(P=0.29)。结论连续交锁缝合可使羊膜紧贴眼表,防止其融解脱落,并促进眼表创面上皮化,其眼表刺激症状很小,可明显提高羊膜眼表覆盖的疗效。Objective To evaluate the efficacy of amniotic membrane transplantation with continuous lock stitch suture for diseases of ocular surface. Method Amniotic membrane transplantation with continuous lock stitch suture was performed in 31 eyes of 26 patients for diseases of ocular surface (group 1), Amniotic membrane transplantation with interrupted sutrue of simple continuous suture was performed in 34 eyes of 3 l patients for diseases of ocular surface (group 2). Results In group 1,Amniotic membrane desquamated in 3 of 31 eyes within 3 weeks post-operatively, and persisted in 19 of 22 eyes after 3 weeks post-operatively, in 17 of 28 eyes corneal epithelialization within 2 weeks post-operatively. In group 2, Amniotic membrane desquamated in 15 of 34 eyes within 3 weeks post-operatively, and persisted in 6 of 21 eyes after 3 weeks post-operatively, in 12 of 32 eyes corneal epithelialization within 2 weeks post-operatively. Conclusions Amniotic membrane transplantation with continuous lock stitch suture can prevent amniotic membrane from desquamated and absorption, promote the epithelialization, reduce pain and increase the efficacy of amniotic membrane transplantation.
