摘 要:目的探讨胰岛素样生长因子ⅡmRNA结合蛋白IMP1在食管癌和贲门癌及癌前病变组织的表达变化特征及其生物学意义。方法采用免疫组化ABC法检测123例食管和贲门癌及癌前病变组织IMP1蛋白的表达状况和特征。结果食管:IMP1在正常鳞状上皮组织表达最高(100%),并随病变进展,阳性表达率呈下降趋势,正常食管上皮与基底细胞过度增生(BCH,91·67%)、不典型增生(DYS,85·71%)和鳞状细胞癌(SCC,83·33%)之间差有异显著性(P<0·05),且BCH与SCC之间阳性表达率差异明显(P<0·05)。贲门:IMP1在各级病变组织均有不同程度表达,不典型增生组织(DYS,83·33%%)阳性率最高,表达最低是慢性萎缩性贲门炎组织(CAG,54·55%),DYS与正常贲门腺上皮(61·11%)、CAG和贲门腺癌(GCA,61·90%)之间差异有显著性(P<0·01)。IMP1表达率与食管鳞癌和贲门腺癌分化程度无相关性(P>0·05)。结论IMP1蛋白表达变化是食管癌、贲门癌和癌前病变组织常见的分子事件,可能在食管鳞癌和贲门腺癌癌变过程中起一定作用。Purpose To evaluate the significance of IMP1 protein expression in esophageal cancer, gastric cardia cancer and precancerous lesions. Methods Immunohistochemical staining ( ABC method) was undertaken to determine the expression of IMP1 proteins in 123 cases of esophageal cancer and gastric cardia cancer and precancerous lesion. Results Esophagus : the positive rate of IMP1 protein was the highest in normal esophageal epithelia( NOR, 100% ) , which positive rate declined in the progress of lesion, and there was significant difference(P〈0.05) between NOR and based cell hyperplasia (BCH,91.67%), NOR and dysplasia (DYS, 85.71% ), NOR and squamous cell carcinoma( SCC, 83.33% ), BCH and SCC, respectively. Gastric cardia: IMP1 expressed in all grades of lesions. 83.33% in dysplasia (DYS), 54. 55% in chronic atrophy gastritis (CAG), and there was significant difference be- tween DYS and normal epithelia(61.11% ) (P 〈0. 05), CAG and GCA(61.90% ) (P 〈0.01 ). And there was not significant relationship between the positive rate of IMP1 and differentiation extent of SCC and GCA ( P〉0.05 ). Conclusions The present results demonstrate that the change of IMP1 protein expression is a frequent molecular event in esophageal and gastric cardia precancerous and cancerous lesions, which indicates that the alterations of IMP1 may play an important role in esophageal and gastric cardia carcinogenesis.
