摘　　要：目的分析三维适形放射治疗对脊椎转移癌的应用价值。方法全组22例,其中男14例,女8例。年龄48～75岁。原发癌为非小细胞肺癌8例、乳腺癌6例、前列腺癌和甲状腺癌各3例,2例为肾癌转移。采用三维适形放疗,根据CT显示病变范围设定临床靶区,设5～6个共面照射野,根据剂量分布决定每个照射野的权重。10%脊髓体积剂量不超过15%。中位剂量36Gy/8次(平均35～40Gy/7～10次)。治疗期间配合适量激素。结果止痛效果明显,总有效率达100%。截止2005年2月,有随访结果的18例,其中8例生存1年,4例存活2年,2例存活3年,有4例存活4年健在。结论三维适形照射技术对脊椎转移癌是一种较为理想的治疗方法。Objective To analyze the result of Three-dimensional conformal radiotherapy for spinal metastatic cancer. Methods 22 cases, include 8 cases of non small cell long cancer, 6 cases of breast cancer, 3 cases of prostate, 3 cases thyroid cancer and 2 cases of kidney cancer,were treated by three-dimensional conformal radiotherapy with a median dose of 36Gy/8 days （35Gy-40Gy）/8-10 days. Results Pain relief rate is 100%. Among the 18 follow-up patients, 8 cases survived 1 year, 4 cases survived 2 years, 2 cases survived 3 years and 4 cases survived 4 years. Conclusions Three-dimensional conformal Radiotherapy is an effective treatment method for spinal metastatic cancer.
