摘  要:目的通过检测人乳头瘤病毒(HPV)16/18型在非小细胞肺癌中的感染状况,初步探讨高危型HPV感染与肺癌发生的关系。方法采用聚合酶链反应(PCR)检测44例非小细胞肺癌及18例肺良性病变组织中HPV16/18两型DNA相关序列,同时采用免疫组织化学SP法检测HPV16/18E6癌蛋白表达情况。结果非小细胞肺癌组及肺良性病变组中高危型HPVDNA阳性率分别为43.18%(19/44)和5.56%(1/18),差异有显著性意义(P〈0.05)。肺癌组E6癌蛋白的检出率为36.36%(16/44),对照组未发现阳性病例(0/18),差异有显著性意义(P〈0.05)。统计分析显示:高危型HPV感染与肿瘤临床分期(Ⅰ~Ⅱ期)、吸烟史有关,与其他因素(年龄、性别、病理分型、肿瘤发生部位、淋巴结转移)无关。结论非小细胞肺癌的发生可能与HPV16/18型感染有关;免疫组化检测E6癌蛋白简便可信,可作为检测高危型HPV感染的常规方法。Objective To investigate the effects of human papillomavirus (HPV) infection in the pathogenesis of non-small cell lung cancer (NSCLC), Methods Forty-four cases of NSCLC and 18 cases of lung benign diseases were analyzed for the presence of HPV type 16 and 18 by polymerase chain reaction (PCR). Immunohistochemistry was used to detect the expression of HPV16/18 E6 in NSCLC and lung benign tissues. Results 43. 18 % (19/44) of NSCLCs and 5.56 %(1/18) of benign diseases were positive for HPV16 or 18 DNA with the difference being significant (P〈0.05). The positive rate of E6 in 44 cases of NSCLC and 18 cases of benign diseases was 36.36 % (16/44) and 0 respectively (P〈0.05). Statistical analysis indicated that clinical stage ( Ⅰ-Ⅱ ) and tobacco usage were associated with the presence of high risk papillomavirus in the tumor. There was no significant association between HPV presence and other factors including age, sex, pathology type, differentiation, site of tumor and lymph node metastasis. Conclusion High risk HPV infection may play an important role in the pathogenesis of NSCLC. Detection of E6 oncoprotein by immunohistochemistry may be a general technique to detect the presence of high risk HPV.
